Management of essential hypertension
Essential hypertension is a condition characterized by persistently high blood pressure; this medication helps regulate blood pressure within normal ranges to protect cardiovascular health. Regular use may reduce the risk of complications such as heart disease and stroke related to hypertension. It also dilates blood vessels to improve blood circulation and lessen the overall burden on the body. This medication is most effective when used alongside lifestyle changes. It is used to alleviate symptoms of essential hypertension and to prevent complications.
The recommended starting dose is 10 mg once daily, administered regardless of meals. If blood pressure is not adequately controlled at this dose, it may be increased to 20 mg once daily. For additional blood pressure reduction, it may be increased to a maximum of 40 mg once daily. The blood pressure-lowering effect appears within 2 weeks of starting treatment, with maximal effect occurring in about 8 weeks. If blood pressure is not adequately controlled with monotherapy, it can be combined with thiazide diuretics such as hydrochlorothiazide. To improve adherence, it is recommended to take it at the same time each day (e.g., in the morning). For patients who may experience intravascular volume depletion (e.g., those receiving diuretics, especially with kidney impairment), treatment should be started at a lower dose with close monitoring of their condition. <For the elderly> There is no need for dose adjustment when administering this medication to patients aged 65 years and older. Although no overall differences in efficacy and safety have been observed between elderly and younger patients, some elderly patients may exhibit a more sensitive response. <For patients with kidney impairment> The maximum recommended dose for patients with mild to moderate renal impairment (creatinine clearance 20-60 mL/min) is 20 mg once daily. <For patients with liver impairment> No dose adjustment is needed for mild liver impairment. The recommended starting dose for moderate liver impairment is 10 mg once daily, with a maximum increase to 20 mg once daily. Experience with severe liver impairment is lacking, so this medication is not recommended for use in such patients. <For children> Safety and efficacy for pediatric patients have not been established.

This medication can cause serious harm or death to the fetus and newborn when taken during the second and third trimesters of pregnancy, so it should be discontinued immediately upon confirmation of pregnancy. Patients allergic to any of the components of this drug, pregnant or breastfeeding women, patients undergoing kidney dialysis, patients with severe liver disease, and patients with bile duct obstruction should not take this medication. Patients with reduced blood volume or salt deficiency may experience symptoms of low blood pressure, so the condition should be thoroughly checked before starting treatment, and appropriate measures should be taken immediately if low blood pressure occurs. This medication can affect kidney function, so patients with kidney disorders or kidney transplant recipients should regularly monitor kidney function and electrolyte levels through blood tests. Due to the blood pressure-lowering effect, dizziness may occur, so caution is advised during hazardous activities such as working at heights or driving. During treatment, caution is needed when taking potassium-sparing diuretics or potassium supplements as blood potassium levels may increase. Use with non-steroidal anti-inflammatory drugs may increase the risk of kidney function deterioration, so patients should consult their doctor and regularly check fluid intake and kidney function. Using this medication during pregnancy may cause serious problems such as impaired kidney function in the fetus, low amniotic fluid, and craniofacial deformities; therefore, it is crucial to inform healthcare providers if planning to become pregnant or if pregnant. Breastfeeding women should be cautious as this medication may pass to breast milk, and the baby could exhibit adverse reactions; it is recommended to either stop breastfeeding or discontinue the medication. Safety and efficacy for pediatric patients have not yet been established, so it is advisable for children not to take this medication. In case of overdose, symptoms such as low blood pressure, palpitations, and bradycardia may occur; immediate medical assistance should be sought, and appropriate emergency treatment may be required. This medication is known to have a low risk of carcinogenicity or genotoxicity, even with prolonged use; however, any unusual symptoms during use should prompt immediate consultation with healthcare providers.
