Improvement of essential hypertension
Essential hypertension is a chronic disease characterized by persistently high blood pressure without a specific cause. This medication reliably lowers blood pressure, reducing strain on the heart and blood vessels. Regular use can also help prevent complications. Therefore, it is used for the treatment of essential hypertension.
The recommended initial dose is 10mg once daily, taken regardless of meals. If blood pressure is not adequately controlled at this dosage, it may be increased to 20mg once daily. The maximum dose may be increased up to 40mg daily. The antihypertensive effect appears within two weeks after treatment initiation, with maximum effect observed in about eight weeks. For patients with possible intravascular volume depletion, treatment should start at a lower dose.
The safety and efficacy in pediatric patients have not been established.
This medication can cause severe harm, including injury or even death to the fetus or newborn if taken during the second or third trimester of pregnancy, so it should be stopped immediately upon confirmation of pregnancy. Individuals allergic to any components of this drug, those who are pregnant or breastfeeding, those undergoing kidney dialysis, those with severe liver impairment, those with biliary obstruction, and those with lactose intolerance should not take this medication. Individuals with reduced blood volume or low sodium levels may experience hypotension when starting this medication and should be monitored closely. If hypotension occurs, it is advised to lie down and rest and, if necessary, administer normal saline. Patients with severe heart failure or impairment of kidney function should use this medication with caution due to the risk of worsening kidney function, and blood potassium levels and kidney function should be regularly monitored. Due to the risk of hyperkalemia, especially in patients with heart or kidney disease, serum potassium levels should be frequently checked. Since this medication lowers blood pressure, dizziness may occur, so caution is required when driving or operating dangerous machinery. While taking this medication, be cautious, as combining it with potassium-sparing diuretics or potassium supplements can increase blood potassium levels, and taking it with other antihypertensive medications can enhance blood pressure-lowering effects. The combination of lithium with this drug is not recommended, as it can raise lithium levels and cause side effects. Taking non-steroidal anti-inflammatory drugs (NSAIDs) in conjunction with this medication can increase the risk of kidney function deterioration, necessitating regular kidney function tests. Taking this drug during pregnancy can lead to serious issues such as decreased fetal kidney function, oligohydramnios, and malformations, so it should not be taken during pregnancy. For breastfeeding, discontinuation of either the medication or breastfeeding is advised to avoid transmission to the baby. Safety and efficacy in pediatric patients have not been established, thus precaution is advised when administering to children. Overdose may lead to hypotension and palpitations, and severe symptoms warrant immediate medical attention. While taking this medication, regular liver function and blood tests should be performed to check for adverse reactions; if severe diarrhea or abdominal pain occurs, consult with a healthcare provider immediately.
