This medication is used to treat essential hypertension. Additionally, it helps reduce the risk of myocardial infarction, stroke, and death from cardiovascular disease in patients aged 55 and older who are at high risk for cardiovascular disease. It can be effectively prescribed even in cases where ACE inhibitors cannot be used. This efficacy concurrently improves hypertension management and reduces cardiovascular-related risks.

How to take

Adult dose

For essential hypertension, Telmisartan should be administered orally at a dose of 40 mg once daily with water, regardless of meal timing. Depending on the patient, a dose of 20 mg once daily may also be effective, and it can be increased to 80 mg once daily if necessary. The maximum antihypertensive effect typically occurs 4 to 8 weeks after starting treatment. For the purpose of reducing cardiovascular disease risk, 80 mg should be taken once daily with water.

Precautions

This medication can cause serious harm to the fetus and newborn during pregnancy, so it should be discontinued immediately if pregnancy is confirmed. This medication should not be used by those with allergies to it or its components, by pregnant or breastfeeding women, or by individuals with severe liver disease or biliary obstruction, hereditary angioedema, or moderate to severe renal impairment among diabetics. Caution is advised for elderly patients, those with high potassium levels, mild to moderate liver impairment, patients with heart valve disease or ischemic heart disease, gastrointestinal disease patients, renal artery hypertension patients, and those with severe renal dysfunction. During the course of treatment, various side effects may present, including infections, blood abnormalities, hyperkalemia, hypoglycemia, mental and neurological disturbances, cardiac rhythm disturbances, hypotension, dyspnea, gastrointestinal disorders, liver function abnormalities, skin rash, myalgia, or renal failure; if any unusual symptoms arise, medical consultation should be sought promptly. In particular, if symptoms of angioedema or severe hyperkalemia, worsening renal function, shock, or fainting occur, medication should be stopped immediately, and appropriate treatment should be received. Patients with reduced renal function should have potassium and creatinine levels checked regularly, as hypotension may occur with reduced blood volume, necessitating caution. Furthermore, when used in conjunction with other antihypertensives, certain diuretics, lithium, non-steroidal anti-inflammatory drugs (NSAIDs), interactions may occur, so it is essential to inform healthcare providers of all current medications. Women who are pregnant or planning to become pregnant should not take this medication, and breastfeeding women should avoid using it as well. Safety and efficacy in children under the age of 18 have not been established, so it is not recommended. Elderly individuals should be observed closely for a potential risk of stroke due to excessive blood pressure lowering, and careful administration is advised. In the event of overdose, symptoms such as hypotension, cardiac rhythm disturbances, dizziness, and acute renal failure may occur, and immediate medical attention should be sought. This medication should be stored out of reach of children and care should be taken with storage and handling, such as opening the packaging just before use. Caution is also needed when driving or operating machinery, as dizziness or fainting may occur.
