Treatment of obesity (weight loss and maintenance); prevention of weight regain
This medication is used for obese patients with a body mass index (BMI) of 30 kg/m² or higher, or for patients with a BMI of 27 kg/m² or higher accompanied by risk factors such as hypertension, diabetes, or dyslipidemia. It helps to reduce body weight or maintain weight in conjunction with a low-calorie diet and reduces the risk of weight regain. Therefore, this medication is effectively used for the treatment of obesity and weight management.
The recommended dose of this medicine is 120 mg of orlistat orally 3 times daily, with a fat-containing meal or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly hypocaloric diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrates, and protein should be distributed evenly across three meals. This medicine may be omitted if a meal is skipped or if the meal contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutrition. The vitamin supplement should be taken at least 2 hours before this medicine or at least 2 hours after it, such as at bedtime. Taking more than 120 mg of this medicine 3 times daily does not increase its effectiveness. Based on measurements of fecal fat, the effect of this medicine appears 24 ∼ 48 hours after administration. After discontinuation of treatment, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine have not been evaluated for periods exceeding 4 years. Studies have not been conducted in patients with hepatic and/or renal impairment or in elderly patients.
Studies have not been conducted in children under 12 years of age.
This medication should not be used in patients with chronic malabsorption syndrome or biliary obstruction. Additionally, it should not be given to individuals who are allergic to this medication or its ingredients, pregnant or potentially pregnant women, and nursing mothers. Liver damage has been reported rarely; therefore, it should be used cautiously in patients with liver function problems, and if symptoms of liver dysfunction occur, the physician should be notified immediately. Patients with hyperoxaluria or a history of calcium oxalate stones should use this medication carefully. This medication may cause various gastrointestinal adverse reactions, including abdominal distension, constipation, fatty stools, fecal incontinence, and frequent bowel movements. These symptoms typically appear within three months of starting treatment and are mostly mild and transient. However, in some patients, they may persist for more than six months, so caution is warranted. Instances of hepatitis and severe liver damage have been reported, so if symptoms such as decreased appetite, itching, jaundice, changes in urine color, or upper abdominal pain occur, it is crucial to consult healthcare professionals immediately. This medication may aid in weight loss and could impact blood sugar control in diabetes patients, potentially requiring adjustments to blood sugar-lowering agents or insulin doses. Additionally, excessive weight loss may increase the risk of gallstones. During treatment, it is advisable to avoid high-fat meals and to distribute fat intake evenly across meals. Moreover, this medication may reduce the absorption of fat-soluble vitamins (A, D, E, K) and beta-carotene, so taking vitamin supplements concurrently is recommended. Vitamin supplements should be taken at least 2 hours before or 2 hours after taking this medication. When taken with cyclosporine, this medication may lower the blood concentration of cyclosporine, so these two medications should be spaced apart, and cyclosporine levels should be monitored frequently. When used in conjunction with anticoagulants (such as warfarin), the coagulation status should be monitored periodically, and patients with a history of seizures should be observed closely for seizure occurrence. Pregnant or nursing women are advised not to take this medication, and it is not recommended for use in children under 12 years old. In cases of overdose, serious side effects are rare, but 24-hour observation is necessary, and if abnormal symptoms occur, healthcare professionals should be notified immediately. Finally, this medication should be stored out of reach of children and kept away from direct sunlight and moisture.
Not everyone experiences these, and they can vary from person to person.
This medication should be stored in a tightly closed container that protects it from air, light, and moisture. It is important to maintain a storage temperature of 25 degrees Celsius or lower. This way, the medication's efficacy is preserved for a long time, preventing degradation.
