This medication is used in the treatment of essential hypertension. Additionally, it assists in reducing the risk of myocardial infarction, stroke, and cardiovascular mortality in patients aged 55 or older who cannot use ACE inhibitors due to high risk of cardiovascular disease. It is particularly effective in patients with a history of coronary artery disease, peripheral artery disease, stroke, or those with high-risk diabetes. Therefore, this medication is used to simultaneously improve the risk of hypertension and cardiovascular complications.

How to take

Adult dose

For the treatment of essential hypertension, Telmisartan should be administered orally at a dose of 40 mg once daily with water, and can be taken regardless of meals. It may also be effective at a dose of 20 mg once daily depending on the patient, and can be increased to a maximum dose of 80 mg once daily if needed. The maximum antihypertensive effect is observed after 4 to 8 weeks of treatment initiation. For the reduction of cardiovascular disease risk, it is recommended to take 80 mg orally once daily with water.

Precautions

This medication can cause severe harm, especially to the fetus and newborn, during pregnancy; therefore, it should be discontinued immediately upon confirmation of pregnancy. It should not be used in individuals with allergies to this medication or its components, pregnant or breastfeeding women, patients with severe liver dysfunction, certain biliary disorders, hereditary angioedema, and specific kidney disorders. Caution should be exercised when used in the elderly, patients with hyperkalemia, patients with impaired liver function, patients with heart valve disease, patients with cerebrovascular disorders, patients with gastrointestinal disorders, and patients with renal vascular hypertension, and their condition should be monitored regularly. During treatment with this medication, various side effects may occur, such as infections, blood abnormalities, hyperkalemia, hypoglycemia, nervous system disorders, cardiovascular disorders, gastrointestinal disturbances, liver dysfunction, skin abnormalities, muscle and joint pain, and kidney dysfunction, so if any unusual symptoms occur, immediate consultation with healthcare professionals is necessary. Symptoms such as a sudden drop in blood pressure, shock, syncope, and loss of consciousness may occur, and immediate treatment is required if these symptoms arise, especially for users of diuretics or patients undergoing dialysis as they are at higher risk of hypotension. Kidney function and serum potassium levels should be regularly monitored, and hydration and electrolyte balance should be well managed. Additionally, it is advisable not to administer this medication within 24 hours prior to surgery, and caution is necessary if gastrointestinal disorders are present due to the risk of gastrointestinal bleeding. This medication may enhance hypotensive effects when used with other antihypertensive agents, and the risk of renal dysfunction or hyperkalemia may increase when used in conjunction with certain drugs; therefore, co-administration should follow healthcare provider recommendations. It is preferable for pregnant or breastfeeding women not to use this medication, and breastfeeding should be discontinued if necessary. Safety and effectiveness in children under 18 years have not been established, therefore it is not recommended.
