Treatment of hypercholesterolemia (primary, familial) and reduction of cardiovascular disease risk
This medication is used when dietary and exercise control is insufficient for patients with primary hypercholesterolemia and mixed hyperlipidemia. It is also administered as an adjunct to diet for patients with familial hypercholesterolemia, particularly among children over 10 years of age. It lowers total cholesterol and LDL-cholesterol levels, delaying the progression of atherosclerosis, and is also beneficial for patients with primary dysbetalipoproteinemia. Furthermore, in adults with elevated high-sensitivity C-reactive protein levels and cardiovascular risk factors, it is used to reduce the risks of stroke, myocardial infarction, and vascular revascularization. Thus, this drug is effective in improving hypercholesterolemia and various cardiovascular risk conditions.
This medication can be taken at any time of the day regardless of meals. The initial dose is 5 mg once daily, and the maintenance dose is usually 10 mg once daily for most patients. The maintenance dose may be properly adjusted at intervals of 4 weeks or more based on LDL-cholesterol levels, treatment goals, and patient responses, and can be increased up to a maximum of 20 mg daily.
Pediatric patients aged 10 to 17 typically use a daily dose ranging from 5 mg to 20 mg. The dose is adjusted at intervals of 4 weeks or more based on each individual patient's treatment goals.
This medication should not be used in specific patients. First and foremost, individuals with allergic reactions to this drug should not take it. It should also be avoided in cases of active liver disease or significantly elevated liver enzyme levels. Patients with muscle disorders or those taking cyclosporine, as well as those with severe renal impairment, should not use this medication. Pregnant or nursing women, and women of childbearing age who do not employ appropriate contraception, should not take this drug, and women who may become pregnant should consult a doctor for careful consideration. Additionally, patients at high risk of myopathy or rhabdomyolysis should avoid high doses. Since this drug contains lactose, patients with lactose intolerance or related genetic conditions should refrain from taking it. Caution is advised when administering this drug to patients with liver function problems or those consuming alcohol heavily. Regular liver function tests before and after taking this medication are recommended. Patients with hypercholesterolemia due to hypothyroidism or nephrotic syndrome need to treat the underlying condition first. Muscle pain or myopathy, and rarely severe muscle damage such as rhabdomyolysis may occur, so patients should report any muscle-related symptoms to their doctor immediately. In particular, in the presence of muscle pain, weakness, or cramps, blood tests should be conducted to check for muscle damage and the medication should be discontinued if necessary. It is safest to discontinue this medication in patients with acute severe illness, post-surgical conditions, or severe metabolic disorders. Rarely, this medication can cause severe muscle weakness or ophthalmoplegia; therefore, it should be stopped immediately if such symptoms appear. When used in combination with other medications, the risk of muscle damage may increase, especially when taken with fibrates, so caution is necessary, and high-dose use is prohibited. This drug may have a transient effect on renal function, so periodic renal function tests are advisable when taken in high doses. In Asians, there may be increased systemic exposure to this drug, necessitating a dosage adjustment. In pediatric and adolescent patients, the impact on growth and sexual development is limited, hence care is required. Rare pulmonary diseases such as interstitial lung disease may develop, so if symptoms like dyspnea or persistent cough arise, consult a doctor immediately. If severe liver damage symptoms or jaundice occur during treatment, discontinue the drug immediately. The risk of diabetes may increase, but because the cardiovascular benefits outweigh this, do not discontinue and maintain strict blood glucose control. The drug response and side effect risk may differ based on genetic factors, so dosage adjustments may be necessary if needed. Severe skin reactions may occur, so if symptoms like skin rash, itching, or fever appear, discontinue the drug immediately and consult a doctor. There is no specific antidote for overdose, so symptomatic treatment and monitoring of liver function and muscle enzyme levels may be necessary. This drug does not significantly impact driving or operating machinery; however, caution should be taken due to potential dizziness. Pregnant or nursing women should not use this drug, and women planning a pregnancy must ensure adequate contraception.
Urticaria, hypersensitivity reactions including angioedema, rhabdomyolysis, Stevens-Johnson syndrome, eosinophilia with systemic symptoms associated drug reaction syndrome (DRESS), severe muscle weakness, ophthalmoplegia, jaundice, hepatitis, acute renal failure, oliguria, severe edema, chest tightness, difficulty breathing, dyspnea, severe allergic reactions, serious skin adverse reactions, respiratory issues, liver abnormalities (jaundice), severe edema or urination problems.

This medication should be stored in an airtight container to protect it from air and moisture, and it is advisable to keep it in a cool place below 30 degrees Celsius. Storing it this way helps maintain its effectiveness and prevents degradation.
