Improvement of symptoms of acute coronary syndrome and atherosclerosis; prevention of thromboembolism in atrial fibrillation
This medication is used to improve atherosclerotic symptoms such as ischemic stroke, myocardial infarction, and peripheral arterial disease. Additionally, it reduces the risk of death, myocardial infarction, and stroke due to cardiovascular abnormalities in patients with acute coronary syndrome who have undergone or will undergo drug therapy, coronary intervention, or coronary bypass surgery, thereby improving atherosclerotic symptoms. Furthermore, it can be safely used in atrial fibrillation patients with vascular risk factors, where vitamin K antagonist treatment is unsuitable and the risk of bleeding is low, reducing the risk of stroke and thromboembolism. It is effective in lowering the risk of complications associated with various cardiovascular diseases and improving symptoms.
○ Adults 1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), start with a loading dose of 300 mg of this drug once daily on the day treatment is initiated, followed by an oral maintenance dose of 75 mg once daily. Aspirin 75 ~ 325 mg should be administered concomitantly with this drug once daily. 3. For patients with atrial fibrillation, administer 75 mg of this drug orally once daily. Aspirin 75 ~ 100 mg should be administered concomitantly with this drug once daily. ○ Patients with renal impairment : Treatment experience in patients with renal impairment is limited. ○ Patients with hepatic impairment : Treatment experience in patients with moderate liver disease and a bleeding diathesis is limited. This drug may be administered with or without food.
This medication helps prevent platelets from clumping together, thereby aiding in blood flow. It may be less effective in individuals with a genetic deficiency of a specific enzyme (CYP2C19), in which case alternative treatment should be considered. Additionally, co-administering this medication with drugs that activate specific enzymes may increase the risk of bleeding, so it is advised not to use them together. This medication should not be used in patients with bleeding disorders, severely impaired liver function, breastfeeding women, or those with certain genetic disorders such as lactose intolerance. Patients at high risk of bleeding or scheduled for surgery should use this medication cautiously, and may need to discontinue it a certain period before surgery. If bleeding symptoms occur during treatment with this medication, the physician should be consulted immediately. This medication may interact with various other drugs, especially when taken with those that increase bleeding risk, thus caution is required. During pregnancy, the effects on the fetus should be considered, and it should be used only when necessary; in breastfeeding cases, careful consideration should be given to whether to continue breastfeeding while taking this medication. The safety and efficacy for children have not yet been adequately established. Elderly patients or those with decreased kidney function may be at higher risk for bleeding, and dosage adjustments and caution are necessary. In cases of overdose, the risk of bleeding may increase, and as there is no specific antidote, treatments such as platelet transfusions may be necessary in cases of severe bleeding. This medication should be stored out of reach of children and not transferred to other containers. During treatment, it may take longer for bleeding to stop, so if bleeding persists or abnormal symptoms occur, medical personnel should be informed immediately.
