Summary

Main use
Effectiveness in treating essential hypertension and heart failure (intolerance to ACE inhibitors); reduces the risk of death after myocardial infarction
Who takes it
Adults · Children
Adult dose
1. Essential Hypertension Take orally with water during meals or between meals. It is recommended to take it at the same time every day (e.g., in the morning). Adults: The recommended oral dosage is 80 mg of valsartan once daily. Patients whose blood pressure is not well controlled may increase the dosage up to 160 mg daily or combine with a diuretic. If further blood pressure reduction is needed, the maximum dosage may reach 320 mg daily. 2. Heart Failure Adults: The initial dose is recommended as 40 mg of valsartan taken orally twice daily, which may be increased to 80 mg or 160 mg once per dose. When combined with diuretics, consider reducing the diuretic dosage. The maximum daily dose is 320 mg to be taken in divided doses. This medication is not recommended for triple combination therapy with ACE inhibitors and beta-blockers. 3. Reducing Mortality Risk after Myocardial Infarction Adults: Treatment may begin within 12 hours after the onset of myocardial infarction. It is recommended to start with 20 mg of valsartan orally, taken twice daily, with careful dosage increases over the next few weeks to 40 mg, 80 mg, and 160 mg, based on patient tolerance. Consider reducing the dosage if symptoms of hypotension or renal impairment occur. This drug may be administered to patients under treatment with other medications (e.g., thrombolytics, acetylsalicylic acid, beta-blockers, and statins) after myocardial infarction. However, combined use with ACE inhibitors is not recommended. 4. Renal Impairment: No dosage adjustment is necessary for those with a creatinine clearance of 10 mL/min or more, but there is no experience with patients with creatinine clearance below 10 mL/min or those on dialysis, so the use of this drug is not recommended. 5. Liver Impairment: For patients with mild to moderate liver impairment, the daily dosage should not exceed 80 mg. If dosage increases above 80 mg, consider whether the therapeutic benefits outweigh the risks. This drug is not to be administered to patients with severe liver impairment, liver cirrhosis, or cholestasis. 6. Pediatrics: Safety and efficacy have not been established in pediatric patients and adolescents under 18 years of age.

Effectiveness in treating essential hypertension and heart failure (intolerance to ACE inhibitors); reduces the risk of death after myocardial infarction

  • Essential hypertension
  • heart failure
  • left ventricular dysfunction
  • left ventricular systolic dysfunction
View approved original text

This medication is used to treat essential hypertension. It is also effective for heart failure patients who cannot use ACE inhibitors, especially those classified as NYHA class II to IV. After myocardial infarction, it helps reduce the risk of death in stable patients with left ventricular dysfunction or left ventricular systolic dysfunction. Hence, it plays a significant role not only in controlling hypertension and managing heart failure but also in improving survival rates after myocardial infarction.

Precautions

This medication can cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters, so it should be stopped immediately upon confirmation of pregnancy. Women who are pregnant or breastfeeding, and people allergic to this drug or its components, should not take it. It is also not safe for patients severely impaired in liver or kidney function, or with a history of angioedema. Elderly patients, those with hyperkalemia, mildly impaired liver function, and those with cardiovascular diseases should use this medication cautiously, starting at a lower dose and monitoring their condition closely. This drug lowers blood pressure and may cause a rapid drop in blood pressure when first taken; if symptoms like dizziness or fainting occur, consult a doctor immediately. During treatment, it's important to regularly monitor blood pressure, as well as kidney function and blood potassium levels. When taken concurrently with other antihypertensives, diuretics, potassium-sparing diuretics, and some anti-inflammatory pain relievers, interactions may occur, so be sure to inform a physician or pharmacist. Caution is required when driving or operating machinery due to the possibility of dizziness. In case of an overdose, symptoms like hypotension or altered consciousness may occur, requiring prompt medical attention. Safety for children and adolescents has not been established; therefore, it's advised not to use this medication in these populations. This drug should be kept out of reach of children and should not be transferred to another container.
