Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), reduction of mortality risk after myocardial infarction.
This medication is used to reduce the risk of death in patients with essential hypertension, heart failure who cannot use ACE inhibitors, and patients with reduced left ventricular function after myocardial infarction. It helps effectively control hypertension, improve cardiac function, and reduce the risk of death from heart disease. Therefore, it plays an important role in the management of hypertension and heart failure, as well as recovery after a myocardial infarction.
Essential hypertension: The recommended dose is 80 mg orally once a day, and if blood pressure control is difficult, it may be increased to 160 mg per day. If further blood pressure reduction is needed, the dose can be increased to a maximum of 320 mg per day. Elderly patients (75 years and older) and patients with specific kidney impairment or fluid depletion should start at 40 mg once daily. Heart failure: 40 mg orally twice daily.
This medication can cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters; therefore, it should be discontinued immediately upon confirmation of pregnancy. Pregnant women, women who may become pregnant, and nursing mothers should not take this medication. Patients with severe liver dysfunction, cirrhosis, or biliary obstruction should not use this medication. Patients with a history of hypersensitivity or angioedema should not take this medication as well. Patients with severe renal impairment or primary hyperaldosteronism should not use this medication. Diabetic patients with moderate or severe renal dysfunction should not combine this medication with certain drugs. Elderly patients, those with high potassium levels, patients with slightly reduced liver function, and patients with heart valve disease or left ventricular hypertrophy should use this medication with caution. Especially those with heart failure or post-myocardial infarction should closely monitor kidney function changes and adjust the dosage gradually as hypotension may occur. Patients undergoing dialysis or under strict sodium restriction should also start at a low dose and closely monitor their condition. This medication can cause a rapid drop in blood pressure, so caution is necessary as syncope or loss of consciousness may occur when taking it for the first time. Due to the risk of hyperkalemia, if taken with potassium-sparing diuretics or potassium supplements, the serum potassium level should be closely monitored. Caution is required when combined with non-steroidal anti-inflammatory drugs (NSAIDs), as it may reduce blood pressure-lowering effects and worsen renal function. When taken with lithium preparations, serum lithium levels may increase, leading to toxicity; thus, monitoring serum levels is necessary. The safety and efficacy of this medication in children and adolescents have not been established, so it is advisable to avoid use in those under 18 years of age. Dizziness may occur while driving or operating machinery, so caution is warranted. In case of overdose, severe hypotension, decreased consciousness, or shock may occur; immediate medical treatment is required.
Hives, severe swelling (edema), angioedema, chest tightness, difficulty breathing, cold sweat, pallor, severe rash with fever, blisters, peeling skin, pain in the mucous membranes of the mouth/eyes, shortness of breath or difficulty breathing, asthma attack, respiratory abnormalities accompanied by cough and fever, jaundice (yellowing of the skin or eyes), severe malaise, sudden and severe headache, thunderclap headache, seizures, altered consciousness, visual disturbances
