Treatment of bipolar disorder (including manic and depressive episodes); prevention of recurrence; treatment of schizophrenia
This medication is used to treat schizophrenia. It is also effective for various symptoms of bipolar disorder, such as the treatment of mania and mixed episodes, prevention of relapse in patients who have responded to manic episodes, and the acute treatment of depressive episodes. Therefore, this drug helps to improve the symptoms of schizophrenia and bipolar disorder.
The therapeutic dose of this drug should be adjusted according to the patient's clinical condition. Any increase beyond the generally recommended daily dose of 10 mg is advised only after an appropriate clinical evaluation. 1. Adults 1) Schizophrenia (1) The recommended initial dose is 5∼10 mg of olanzapine once daily. (2) Maintenance treatment: It is difficult to determine precisely how long treatment with this drug should be continued. Its efficacy was demonstrated in a placebo-controlled study in which patients with schizophrenia whose symptoms had stabilized after taking this drug for approximately 8 weeks received maintenance treatment at a dose of 10∼20 mg daily for 8 months. The need for maintenance treatment at an appropriate dose should be assessed periodically. 2) Bipolar disorder (1) Treatment of manic and mixed episodes associated with bipolar I disorder The initial dose is 15 mg once daily as monotherapy or 10 mg once daily as adjunctive therapy with lithium or valproate. (2) Prevention of recurrence in bipolar disorder The recommended initial dose is 10 mg once daily. Patients who have received this drug to treat mania should continue the same dose for prevention of recurrence. If a new manic, mixed, or depressive episode occurs, treatment with this drug should continue, together with adjunctive treatment for mood symptoms as indicated by clinical signs (with the necessary doses optimized). 3) For schizophrenia, treatment of manic and mixed episodes associated with bipolar I disorder, and prevention of recurrence in bipolar disorder, the daily dose may be adjusted within the range of 5 mg to 20 mg based on clinical status. Increases beyond the recommended initial dose are advised only after an appropriate clinical evaluation, and generally should be made at intervals of at least 24 hours. If treatment with this drug is discontinued, the dose should be gradually reduced with caution. 4) Acute treatment of depressive episodes associated with bipolar I disorder Start treatment with olanzapine at 5 mg once daily, then increase to 10 mg once daily. Take this drug at bedtime; adjust the dose appropriately according to age and symptoms, but do not exceed a daily dose of 20 mg. Food does not affect drug absorption, so this drug may be taken with or without food. 2. Dosage in special populations 1) Elderly patients Starting at a dose of 5 mg or less daily may be considered when supported by sufficient clinical factors. 2) Patients with renal or hepatic impairment A low initial dose (5 mg) should be considered. In moderate hepatic impairment (cirrhosis, Child-Pugh class A or B), the initial dose is 5 mg and should be increased with caution. 3) Patients with a combination of factors that delay metabolism of this drug (female sex, older age, and nonsmoking status) may also be started at a lower dose.
This medication can cause a very dangerous condition known as Neuroleptic Malignant Syndrome; if symptoms such as high fever, muscle rigidity, or changes in mental state occur, you should immediately discontinue use and seek medical attention. It is not suitable for the treatment of psychosis or behavioral disorders related to dementia, and its use may increase the risk of mortality and cerebrovascular events, so special caution is recommended for elderly patients. Long-term use may cause tardive dyskinesia, a movement disorder; if abnormal symptoms appear, consider reducing or discontinuing the dosage. It may take several days to weeks for symptoms to improve during treatment, so careful monitoring of the patient is required. Diabetes or hyperglycemia may develop, so regular monitoring of blood sugar and weight is necessary, and any abnormal symptoms should be discussed with a physician immediately. Suddenly stopping the medication may result in symptoms such as sweating, insomnia, anxiety, nausea, etc., so it is safer to gradually reduce the dosage before stopping. If you have heart disease or are elderly, your blood pressure and heart condition should be monitored regularly, and caution should be taken regarding interactions with other medications. Women who are pregnant or breastfeeding should consult a physician before taking this medication, and if breastfeeding, discontinuing nursing is recommended. This medication may cause drowsiness and dizziness, so caution is advised when driving or operating dangerous machinery. Severe side effects may occur in the case of overdose, so the dosage should be strictly adhered to and used according to the physician's instructions. This medication should be stored out of reach of children and kept in a sealed container away from light and moisture at 15-30 degrees Celsius.
