This medication is used to control blood pressure in patients with essential hypertension. It is also helpful for heart failure patients who are intolerant to ACE inhibitors, especially those classified as NYHA class II to IV. It is effective in reducing the risk of mortality in clinically stable patients with left ventricular dysfunction or impairment of left ventricular contractility after myocardial infarction. Thus, it is beneficial for the management of hypertension, heart failure, and post-myocardial infarction.

How to take

Adult dose

Essential hypertension: The recommended dose is 80 mg orally once daily. If blood pressure control is inadequate, the dose may be increased to 160 mg once daily, or a diuretic can be combined, with a maximum dose of up to 320 mg daily. Elderly patients (75 years or older) should start at 40 mg orally once daily. Heart failure: The recommended dose is 40 mg orally twice daily with possible increases to 80 mg or 160 mg per dose.

Child dose

Safety and efficacy in pediatric and adolescent patients under 18 years have not been established.

Precautions

This medication can cause serious harm to the fetus, especially during the second and third trimesters, if taken during pregnancy; therefore, it should be discontinued immediately upon confirmation of pregnancy. This medication should not be taken by individuals with allergies to this drug or its components, pregnant patients, patients with severe liver disease, or those with significantly impaired kidney function, or those with a history of specific angioedema. Elderly patients, patients with high potassium levels, patients with reduced liver function, or patients with heart valve disease or cardiovascular disease should take this medication with caution and should be closely monitored during treatment. Blood pressure may drop sharply at the beginning of treatment, causing dizziness or fainting; therefore, patients undergoing dialysis or taking diuretics, or those with restricted salt intake should start with a low dose and gradually increase it. This medication may increase potassium levels in the blood; therefore, caution is needed when taken with potassium-sparing diuretics or potassium supplements. Additionally, when used with other antihypertensive drugs or specific anti-inflammatory pain relievers, the blood pressure control effect may be reduced or kidney function may be affected, so it should be taken under the guidance of a physician. During treatment, symptoms such as headaches, dizziness, fatigue, cough, and dyspepsia may occur, and, rarely, severe allergic reactions, kidney function decline, or blood abnormalities may occur, so any unusual symptoms should be reported to healthcare professionals immediately. It is preferable not to take this medication during pregnancy or breastfeeding; if breastfeeding, it is advisable to discontinue nursing while taking this medication. Safety and efficacy in children and adolescents under 18 years of age have not been established, and therefore it is not recommended for use. In case of overdose, severe hypotension or altered consciousness may occur, and immediate medical attention should be sought for proper treatment. This medication should be kept out of reach of children and should not be transferred to other containers for safety.
