Improvement of symptoms of acute coronary syndrome and atherosclerosis; prevention of thromboembolism in atrial fibrillation
This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. It also helps to reduce the risk of death from cardiovascular events such as myocardial infarction and stroke in patients with acute coronary syndrome or those who have undergone coronary interventions or bypass surgery. Additionally, in patients with atrial fibrillation who have vascular risk factors and cannot take vitamin K antagonists, it aids in decreasing the risk of stroke and thromboembolism when the risk of bleeding is low. Thus, it is effectively used for the prevention of various cardiovascular diseases and complications.
○ Adults 1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), begin treatment on the day this medicine is initiated with a loading dose of 300 mg once daily, followed by an oral maintenance dose of 75 mg once daily. Aspirin 75 ~ 325 mg should be administered concomitantly with this medicine once daily. 3. For patients with atrial fibrillation, administer this medicine 75 mg orally once daily. Aspirin 75 ~ 100 mg should be administered concomitantly with this medicine once daily. ○ Patients with renal impairment : Experience with treatment in patients with renal impairment is limited. ○ Patients with hepatic impairment : Experience with treatment in patients with moderate hepatic disease and a bleeding diathesis is limited. This medicine may be administered with or without food.
This medication helps to prevent platelets from aggregating, thereby facilitating blood flow. Individuals with a genetic deficiency in a specific enzyme (CYP2C19) may find that this medication does not work effectively, and they may be more susceptible to cardiovascular issues such as heart attacks. In such cases, alternative treatment options should be discussed with a physician. Moreover, taking this medication with drugs that activate this enzyme may excessively enhance its effects, increasing the risk of bleeding, so concurrent usage is not advised. Do not use this medication in patients with allergies to it, patients with bleeding disorders, patients with severe liver dysfunction, nursing mothers, and patients with genetic issues like lactose intolerance. Patients at risk of bleeding, blood coagulation problems, or those who have undergone recent surgery should use this medication cautiously and should inform their physician immediately of any bleeding symptoms. If surgery is planned, discontinue this medication 5 to 7 days prior. This medication may increase the risk of bleeding, so caution is needed when taken with other anticoagulants (e.g., aspirin, nonsteroidal anti-inflammatory drugs, thrombolytics, specific antidepressants, etc.). While taking this medication, there may be an increased occurrence of bleeding; if there are any bleeding sites or if bleeding durations are longer than usual, inform a physician promptly. During pregnancy, while the risk to the fetus is low, there is a risk of bleeding during delivery, so discontinue the medication 5 to 7 days before the expected delivery date. If nursing, this medication may pass into breast milk, so consult a physician regarding whether to continue breastfeeding or discontinue the medication. Safety and efficacy in children have not been established. Elderly individuals may have decreased bodily functions, leading to a higher risk of bleeding, thus requiring dosage adjustments and close monitoring of their condition. Overdose may increase the risk of bleeding, and as there are no specific antidotes, emergency treatment may involve platelet transfusion. Keep this medication out of the reach of children, and do not transfer it to other containers.
