Summary

Main use
Relief of pain (e.g., during labor and myocardial infarction), preoperative sedation, and symptoms of acute pulmonary edema
Who receives it
Adults · Children
Adult administration
(Injection 5 mg, 8 mg, 10 mg, 15 mg) This medicine must be administered by subcutaneous injection, intramuscular injection, or slow intravenous injection; do not administer epidurally or intrathecally. When administering this medicine intravenously, have resuscitation equipment and an opioid antagonist (such as naloxone) immediately available. For patients with severe chronic pain, adjust the dose according to the severity of pain, response, and tolerance. The dose may be increased in patients with very severe chronic pain or in those who are tolerant to the analgesic effects of opioids. This medicine is primarily intended for subcutaneous injection, but may also be administered by intramuscular injection or slow intravenous injection. Repeated subcutaneous injections may cause local inflammation, pain, and induration; therefore, intramuscular injection is preferred over subcutaneous injection for repeated administration. 1) Dose for subcutaneous or intramuscular injection: The usual dose is 10 mg of morphine sulfate hydrate every 4 hours as needed. The dose per administration may be within the range of 5∼20 mg, depending on the patient's response and requirements. 2) Dose for intravenous injection: The usual dose is 4∼10 mg of this medicine, administered slowly over 4∼5 minutes and repeated every 4 hours as needed. 2.5∼15 mg of this medicine may be diluted in 4∼5 mL of water for injection. 3. Relief of pain during labor: The usual dose is 10 mg of this medicine by subcutaneous or intramuscular injection. 4. Relief of pain due to myocardial infarction: Inject 8∼15 mg of this medicine. If the pain is very severe, small additional doses may be administered every 3∼4 hours.
Child administration
Reduce the dose in children, elderly patients, and patients receiving other central nervous system depressants. Children: Inject 0.1∼0.2 mg/kg of this medicine subcutaneously as needed; do not exceed 15 mg per dose. Do not administer to premature infants. Safety and efficacy in newborns have not been established.

This medication helps alleviate relatively severe acute or chronic pain, such as that associated with terminal illnesses. It is also used as an anesthetic adjunct to aid in the sedation of patients before surgery, and is effective in relieving pain during childbirth. Additionally, it may be utilized to improve cardiovascular function in patients with acute pulmonary edema and to reduce anxiety (not to be used in cases of pulmonary edema caused by chemical respiratory irritants). It is also used for pain relief due to myocardial infarction. This drug is very useful in alleviating pain and anxiety in a variety of clinical situations.

Precautions

This drug has a central nervous system depressant effect; therefore, taking it with alcohol or benzodiazepine medications can lead to severe drowsiness, respiratory distress, coma, and in severe cases, death. This drug should not be used in patients with certain conditions, for example, those who have allergies to this drug or its components, those with severe respiratory distress, patients experiencing persistent asthma attacks, those with cardiac arrhythmias, brain tumor patients, and patients with acute alcohol intoxication. Careful monitoring is necessary when using this drug in patients with hypotension or shock, chronic lung diseases, convulsive disorders, severe liver or kidney dysfunction, elderly patients, and children, as the possibility of side effects needs to be closely observed. Long-term use of this drug can lead to dependence; therefore, abrupt discontinuation can cause withdrawal symptoms, and the dosage should be gradually reduced according to a doctor's instructions. Respiratory depression symptoms may occur; hence, respiratory status should be closely monitored during administration, and if respiratory distress occurs, immediate emergency treatment is required. Patients receiving this drug may experience drowsiness or dizziness, so it is advisable to avoid driving or operating hazardous machinery. Pregnant or breastfeeding women should discuss the use of this drug with their physician, as it may affect the fetus or newborn, with the recommendation to suspend breastfeeding during treatment. The side effects may be exacerbated when used with other central nervous system depressants, so dosage adjustments and monitoring of conditions are necessary when co-administered. An overdose may lead to severe respiratory depression, loss of consciousness, and hypotension, requiring immediate emergency intervention and narcotic antagonists (e.g., Naloxone) if necessary. This drug should be stored in a container that protects it from light and kept in a cool place at room temperature between 1 to 30 degrees Celsius.
