Treatment of psychiatric disorders (schizophrenia and mania), symptoms of Alzheimer's dementia, and disruptive behavior disorders
This medication is used to treat schizophrenia and is also effective for short-term treatment of anxiety, aggression, and psychotic symptoms in moderate to severe Alzheimer's-type dementia patients. Additionally, it is used as an adjunctive treatment for stabilizing mood during manic episodes in bipolar disorder. Furthermore, it is used for the treatment of disruptive behavior and behavioral disorders in children, adolescents, and adults with intellectual disabilities or low intellectual ability. Overall, it helps improve various symptoms of mental and behavioral disorders.
The therapeutic dose of this medication should be adjusted according to the patient's clinical condition, and the lowest effective dose possible should be administered. Place this medication on the tongue. When it disintegrates within a few seconds, swallow it with or without water (for orally disintegrating tablets only). 1. Schizophrenia Adults Initial dose: Administer risperidone 1 mg orally twice daily and increase the dose gradually. Maintenance dose: Administer 1–3 mg twice daily. Adjust the dose appropriately according to age and symptoms. No additional benefit was observed with doses of 10 mg or more per day compared with lower doses, and the risk of extrapyramidal symptoms may increase. Doses of 10 mg or more per day should be administered only when the benefits to the individual patient are judged to outweigh the risks. Do not administer doses of 16 mg or more per day, as safety has not been established. A benzodiazepine may be administered concomitantly if additional sedation is required. Elderly patients and patients predisposed to hypotension: The initial dose is 0.5 mg twice daily. Depending on the patient's condition, increase the dose by 0.5 mg twice daily until a dose of 1–2 mg twice daily is reached. In general, increases to doses of 1.5 mg or more twice daily should be made at intervals of at least 1 week. Switching from existing antipsychotic medication to this medication: When medically appropriate, initiate this medication while gradually reducing the dose of the existing medication. Also, when medically appropriate, when switching from an existing depot antipsychotic, initiate this medication in place of the next scheduled depot injection. The need to continue concomitant antiparkinsonian medication should be reassessed regularly. 2. Agitation, aggression, or psychotic symptoms in patients with dementia of the Alzheimer type The initial dose is 0.25 mg of this medication twice daily. If necessary, depending on the patient, the dose may be increased by 0.25 mg twice daily at intervals of at least 2 days. The optimal dose for most patients is 0.5 mg twice daily, although the dose may be increased to up to 1 mg twice daily depending on the patient. Once the patient has reached the optimal dose, once-daily administration may also be considered. As with symptomatic treatment, the appropriateness of continued use of this medication should be evaluated. 3. Adjunctive treatment of mania in bipolar disorder An initial dose of 2 mg of this medication once daily is recommended. Depending on the patient, the dose may be increased by up to 1 mg per day at intervals of at least 24 hours. The optimal dose for most patients is within the range of 2–6 mg per day. As with symptomatic treatment, the appropriateness of continued use of this medication should be evaluated. Renal and hepatic impairment In patients with renal or hepatic impairment, administer half the initial and maintenance doses and adjust the dose more gradually. Use this medication with caution in these patients (see 5. General Precautions).
Schizophrenia: There is insufficient clinical experience in children and adolescents under 15 years of age. Adjunctive treatment of mania in bipolar disorder: There is insufficient clinical experience in children and adolescents under 15 years of age. Behavioral disorders and other disruptive behavior disorders, body weight 50 kg or more: The initial dose is 0.5 mg of this medication once daily. If necessary, depending on the patient, the dose may be increased by 0.5 mg per day at intervals of at least 2 days. The optimal dose for most patients is 1 mg once daily. Depending on the patient, 0.5–1.5 mg may be administered once daily. Behavioral disorders and other disruptive behavior disorders, body weight less than 50 kg: The initial dose is 0.25 mg once daily. If necessary, depending on the patient, the dose may be increased by 0.25 mg per day at intervals of at least 2 days. The optimal dose for most patients is 0.5 mg once daily. Depending on the patient, 0.25–0.75 mg may be administered once daily. Behavioral disorders and other disruptive behavior disorders: There is insufficient clinical experience in children under 5 years of age.
Caution is required when administering this medication to elderly dementia patients since mortality may be increased in this population. In particular, the risk of death may be higher when used in conjunction with the diuretic drug furosemide, so the risks and benefits should be carefully considered before prescribing. Before using this drug, assess the risk factors for venous thrombosis and take preventive measures if necessary. Additionally, this drug contains the artificial sweetener aspartame, and individuals with certain genetic disorders, such as phenylketonuria, should not take it. It should not be administered to specific patients, for example, those with allergies to this drug or its ingredients, those severely sedated, in a coma, receiving epinephrine, or those with lactose intolerance. This drug should be used cautiously in patients with cardiovascular diseases, impaired liver or kidney function, a history of seizures, patients at risk for suicide, those with Parkinson's disease, diabetic patients, elderly individuals, and pregnant or breastfeeding women. This drug may cause various side effects, such as drowsiness, dizziness, orthostatic hypotension, weight gain, and increased blood sugar levels, so these symptoms should be closely monitored during use. Additionally, this medication may impair mental concentration, so caution is advised when driving or operating machinery. Using this drug concurrently with other central nervous system depressants or alcohol may enhance the central nervous system depressant effects, so caution is needed when combining them. Moreover, it can interact with multiple medications, so it is essential to inform your doctor or pharmacist about any other medications you are taking. During pregnancy or breastfeeding, this medication should only be given when necessary, considering its impact on the fetus or newborn, and discontinuation of breastfeeding is recommended. In case of overdose, seek immediate medical attention for appropriate treatment, as there is no antidote for this drug, and symptomatic supportive care is necessary. Finally, this drug should be kept out of the reach of children, taken immediately after opening, and should not be mixed with tea or cola.
