Summary

Main use
Treatment of hypertension; prevention of cardiovascular events in stable coronary artery disease; adjunctive treatment of congestive heart failure
Who takes it
Adults
Adult dose
The dose should be adjusted according to the patient's condition and blood pressure response. It is recommended that this medicine be taken once daily in the morning before breakfast. This medicine can be split in half (5 mg only). 1. Hypertension 1) Adults Administer 5 mg of perindopril arginine orally once daily in the morning. If necessary, the dose may be increased to 10 mg once daily after 1 month of treatment. 2) Patients with strongly activated renin-angiotensin-aldosterone system (RAAS) (especially those with renovascular hypertension, salt and/or volume depletion, cardiac decompensation, or severe hypertension): Excessive lowering of blood pressure may occur after administration of 5 mg of this medicine. In these patients, an initial dose of 2.5 mg is recommended under a physician's supervision. 3) Older adults Depending on the patient's condition and status, administer an initial dose of 2.5 mg once daily in the morning. If necessary, the dose may be increased to 5 mg once daily after 1 month of treatment. 4) Patients with renal impairment Adjust the dose according to the degree of renal impairment. Measure serum potassium and creatinine, and administer the following doses: Creatinine clearance ≥ 60 mL/min: 5 mg/day 30 <, < 60 mL/min: 2.5 mg/day 15 <. < 30 mL/min: 2.5 mg every other day Patients on hemodialysis* < 15 mL/min: 2.5 mg on dialysis days * The dialysis clearance of perindoprilat, the active metabolite of this medicine, is 70 mL/min. Patients receiving hemodialysis should take this medicine after dialysis. 5) Patients taking diuretics When starting treatment with 5 mg of this medicine, symptomatic hypotension may occur, mainly in patients taking diuretics. These patients may have salt and/or volume depletion, so administer this medicine with caution. If possible, discontinue the diuretic 2 ∼ 3 days before starting treatment with 5 mg of this medicine. In patients for whom discontinuation of the diuretic is not possible, start treatment with 2.5 mg of this medicine. In this case, monitor renal function and serum potassium, and adjust the dose of this medicine according to the blood pressure response. The diuretic may be restarted if necessary. 6) Renovascular hypertension Revascularization is preferable; however, if surgery is planned or cannot be performed, administer an initial dose of 2.5 mg once daily with caution. 2. Congestive heart failure 1) Adults An initial dose of 2.5 mg once daily in the morning is recommended. If well tolerated after 2 weeks, the dose may be increased to 5 mg. Adjust the dose according to the patient's clinical response. 2) Patients with severe congestive heart failure, older adults, patients with very low initial arterial pressure, patients with renal impairment, and patients receiving high doses of diuretics Administer an initial dose of 1.25 mg once daily with close observation and caution. 3. Stable coronary artery disease 1) Adults Administer an initial dose of 5 mg once daily for 2 weeks. Thereafter, if 5 mg is well tolerated, administer 10 mg once daily, taking renal function into consideration. 2) Older adults Administer an initial dose of 2.5 mg once daily for 1 week, followed by 5 mg once daily for the next week. Thereafter, administer 10 mg once daily, taking renal function into consideration. Increase the dose to a higher dose only if the lower dose is well tolerated, taking renal function into consideration. 3) Patients with renal impairment Adjust the dose as follows according to renal function (creatinine clearance): Creatinine clearance ≥ 60 mL/min: 5 mg/day 30 <, < 60 mL/min: 2.5 mg/day 15 <, < 30 mL/min: 2.5 mg every other day Patients on hemodialysis* < 15 mL/min: 2.5 mg on dialysis days * The dialysis clearance of perindoprilat, the active metabolite of this medicine, is 70 mL/min. Patients receiving hemodialysis should take this medicine after dialysis. 4. Patients with hepatic impairment No dose adjustment is necessary in patients with hepatic impairment.

This medication controls blood pressure in patients with hypertension, thus reducing the risk of cardiovascular diseases. Additionally, it is used adjunctively in patients with congestive heart failure along with potassium-sparing diuretics and digitalis to help alleviate symptoms. In patients with stable coronary artery disease who have a history of myocardial infarction or vascular interventions, this drug is effective in reducing the risk of death related to cardiovascular diseases or non-fatal myocardial infarction. In summary, this drug is used for hypertension, congestive heart failure, and stable coronary artery disease.

Precautions

This drug carries a risk of blood abnormalities such as neutropenia or agranulocytosis in patients with weakened immune systems. Caution is particularly needed in patients with systemic lupus erythematosus, scleroderma, or those undergoing immunosuppressive therapy with renal failure. Angioedema may occur on the face, lips, tongue, or in the throat, so if such symptoms arise, discontinue medication immediately and inform medical personnel. In severe cases, airway obstruction could pose a life-threatening risk which requires prompt emergency care. Patients undergoing venom desensitization therapy should use caution as severe allergic reactions may occur with this medication, and if necessary, medication should be paused prior to treatment. This drug can cause severe harm to the fetus if used during pregnancy, so if pregnancy is confirmed, it should be discontinued immediately. Additionally, patients receiving specific hemodialysis treatment, those with a history of angioedema, and those allergic to this drug or similar medications should not use this drug. Patients with impaired renal function, elderly patients, and those with fluid or sodium depletion should start with a low dose and cautiously increase to monitor their condition. During the use of this drug, blood pressure may drop too low, so caution is warranted in the presence of dizziness or drowsiness symptoms, and dose adjustment may be necessary in consultation with medical personnel. Various side effects including cough, dizziness, headache, dyspepsia, rash, muscle cramps, weakness, and changes in mood may appear, and persistent abnormal symptoms should be reported to a healthcare provider. There is a risk of elevated serum potassium levels, so caution is needed when taking with potassium-sparing diuretics or potassium supplements, and potassium levels should be monitored regularly through blood tests. Diabetic patients should closely monitor blood sugar levels, and it is advisable not to take lithium-based medications concomitantly due to the risk of lithium toxicity. Patients scheduled for surgery or anesthesia should discontinue this medication from the day before surgery, and it is important to inform medical staff as hypotension may occur. This drug has not been established as safe or effective for use in children. In the case of overdose, symptoms such as hypotension, shock, and electrolyte abnormalities may occur, requiring immediate medical attention.
