Summary

Main use
Treatment of symptoms of Alzheimer-type dementia
Who takes it
Adults · Children
Adult dose
○ Adults: Administer 5 mg of donepezil hydrochloride once daily at bedtime. If sleep disturbances such as abnormal dreams, nightmares, or insomnia occur, morning administration of this medicine may be considered. Since donepezil concentrations reach steady state 15 days after administration and the frequency of adverse reactions may be affected by the rate of dose escalation, administer the 5 mg dose for 4~6 weeks. After assessing the clinical response during this period, the dose may be increased to 10 mg. When increasing the dose to 10 mg daily, administer with attention to gastrointestinal adverse reactions. If this medicine is discontinued, its effects gradually diminish, and no rebound effect occurs after abrupt discontinuation. ○ Female patients aged 85 years or older with low body weight: Careful monitoring is required because adverse reactions occur more frequently. Elderly female patients with low body weight should not exceed 5 mg per day.
Child dose
○ Children: There is no experience with use in children.

Precautions

This medication contains donepezil hydrochloride and is used to alleviate Alzheimer's disease symptoms. It should not be used by patients who are allergic to donepezil hydrochloride or similar ingredients, pregnant women or women who may become pregnant, and breastfeeding women. Additionally, it contains lactose, so patients with lactose intolerance or certain genetic metabolic disorders should not take it. Patients with heart diseases or conduction disorders should be cautious while taking this medication, as bradycardia or arrhythmia may occur. Patients taking non-steroidal anti-inflammatory analgesics or with a history of peptic ulcer should use it cautiously due to the risk of gastrointestinal bleeding or ulcer deterioration. Patients with asthma or obstructive lung disease should be careful, as bronchoconstriction and increased mucus production may worsen symptoms. Patients with extrapyramidal disorders like Parkinson's disease should also be cautious, as symptoms may worsen. This medication contains the dye Yellow No. 5, which may cause an allergic reaction in sensitive individuals. Serious side effects such as syncope, bradycardia, heart block, gastrointestinal bleeding, liver dysfunction, seizures, muscle symptoms, shortness of breath, acute pancreatitis, and renal failure may occur. If any abnormal symptoms appear, consult a physician immediately. Mild side effects may include nausea, vomiting, diarrhea, dizziness, insomnia, headache, and fatigue, most of which are temporary and improve with dosage adjustment. This medication is approved for use only in Alzheimer's patients and should not be used for other types of dementia or memory disorders. The therapeutic effect should be evaluated regularly, and if there is no effectiveness, discontinuation should be considered. This medication can affect the heart, so patients with a history of heart diseases or those taking specific medications may need to undergo an ECG. Caution is advised when driving or operating machinery, especially in the initial stages of treatment or after a dosage increase, as fatigue and dizziness may occur. It should preferably be avoided during pregnancy, and breastfeeding women are advised to discontinue breastfeeding when taking this medication. Safety in pediatric patients has not been established, so it is not to be used in children. This medication may affect liver and kidney function, requiring regular checks to monitor status. In cases of overdose, symptoms such as nausea, vomiting, bradycardia, respiratory depression, and seizures may appear, so immediate medical attention is necessary. This medication may interact with other drugs, so if you are taking any other medications, be sure to inform your doctor or pharmacist and seek advice. Particular caution is required when used in conjunction with medications that affect the heart, antibiotics, antidepressants, and antipsychotic drugs.
