Summary

Main use
Relief of moderate to severe acute and chronic pain
Who takes it
Adults
Adult dose
For children 12 years and older and adults: The dosage should be adjusted based on the severity of the pain and treatment response. A starting dose of 2 tablets is recommended, and subsequent doses should be at least 6 hours apart, not exceeding 8 tablets in one day. The lowest effective dosage should be given for the shortest duration possible. Do not take this medication for longer than necessary, and if long-term use is required due to the nature of the disease and severity, regular monitoring should be conducted to assess whether to continue this medication. Elderly Typically, administer the usual adult dosage. However, in patients aged over 75, the oral half-life of tramadol increased by 17%, so doses should be given at minimum intervals of 6 hours. Patients with renal impairment For moderate renal impairment (creatinine clearance 10-30 ml/min), dosing intervals should be extended to 12 hours. Severe renal impairment patients (creatinine clearance less than 10 ml/min) are not recommended for treatment. Tramadol is removed very slowly during hemodialysis or filtration, so it is unnecessary to re-administer the medication after dialysis for pain relief. Patients with liver impairment For moderate liver impairment patients, careful consideration should be given to prolong the dosing intervals. For severe liver impairment patients, administration of this medication is not recommended.

Precautions

This medication can put stress on the liver; therefore, individuals who regularly consume more than three glasses of alcohol daily should consult with a doctor or pharmacist before taking it. When taking this medication, there is a risk of serious rash or allergic reaction; if a skin rash or other hypersensitivity reactions occur, discontinue use immediately and consult a physician. This medication contains tramadol, which poses a risk of seizures; caution should be exercised if there is a history of seizures or if taking other medications that can induce seizures. Though rare, anaphylaxis, a serious allergic reaction, may occur while taking this medication; if sudden difficulty breathing or hives appear, seek emergency medical attention immediately. Patients with severe respiratory diseases may find it difficult to breathe while taking this medication, so it should only be administered in a medical setting. Taking this medication with benzodiazepines or alcohol may cause severe drowsiness, respiratory depression, or coma; therefore, caution is advised not to combine them. Tramadol can cause both psychological and physical dependence; therefore, caution is advised for long-term use, and it should not be abruptly discontinued without a doctor's guidance. Patients with liver dysfunction may experience liver damage from the acetaminophen component of this medication and should consult with their physician before use. Particular care should be taken for individuals with a very fast-acting enzyme called CYP2D6, as they may produce excessive active metabolites, which increases the risk of respiratory depression. This medication should not be used in children under 12 or in teenagers under 18 who have undergone tonsillectomy or adenoidectomy, and teens aged 12 to 18 should avoid use if there are special risk factors. This medication may induce sleep apnea and hypoxemia; thus, continuous monitoring for worsening symptoms is necessary, and doses should be reduced or discontinued as needed. Elderly patients or those at risk of hyponatremia should be closely monitored for symptoms of hyponatremia while taking this medication. This medication should not be administered to certain patients, for example, those who are allergic to the components, have severe respiratory depression, have head injuries, or are taking MAO inhibitors. There are many interactions with other medications; those taking other medications should consult with their healthcare provider, especially regarding MAO inhibitors, serotonin agents, CYP3A4 and CYP2D6 related drugs, benzodiazepines, and alcohol. This medication may cause drowsiness and dizziness; caution is advised when driving or operating machinery. In cases of overdose, serious respiratory depression, seizures, and liver damage may occur; therefore, it is essential to adhere to the prescribed dosage, and if overdose is suspected, immediate emergency treatment is required. When storing this medication, keep it in a sealed container that prevents air or moisture from entering, and store it at room temperature between 1 and 30 degrees Celsius to maintain its efficacy safely.
