Summary

Main use
Treatment of herpes zoster virus infection
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster infection Adults: Administer famciclovir 750 mg orally once daily for 7 days. Take it at the same time each day. It is preferable to start treatment as soon as symptoms appear after infection. 2. Patients with renal impairment: Since creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster infection Creatinine clearance (mL/min/1.73㎡) 30~59 : 250 mg per dose, 2 times daily 10~29 : 250 mg per dose, once daily 2) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 3) Patients with renal impairment undergoing hemodialysis: It is preferable to administer the medicine every 48 hours, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer the full dose of this medicine immediately after dialysis. 3. Patients with hepatic impairment: No dose adjustment is necessary for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with evident chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This drug should not be used in certain patients. Patients who have had an allergic reaction to this drug, its components, or the active ingredient penciclovir should not take this medication. Additionally, this drug contains lactose, so patients with a genetic inability to digest lactose should also not take it. Caution is required for patients with impaired kidney function when taking this medication. In cases of decreased kidney function, the excretion of the medication may be difficult, leading to the occurrence of side effects. During treatment with this drug, symptoms such as headache or nausea may appear, but they are mostly mild to moderate in severity. Additionally, there may be rare occurrences of decreased platelet count or reduction in white blood cells and neutrophils. Neurological symptoms such as confusion, drowsiness, dizziness, and hallucinations may occur, and particularly in older adults, drowsiness may be more pronounced. It may affect the gastrointestinal tract, frequently resulting in symptoms such as nausea, vomiting, abdominal pain, and diarrhea. Liver function test values may elevate, and jaundice may rarely occur, so caution regarding liver health is necessary. Skin rash or itching may commonly occur, and in rare cases, severe skin reactions or angioedema, resulting in swelling of the face, around the eyes, or in the neck area, may develop. Acute kidney injury has been rarely reported in patients with poor kidney function, so kidney status should be monitored closely. This drug does not completely treat genital herpes, so even during treatment, if symptoms are present, it is important to avoid sexual contact and maintain safe sexual practices. There is no evidence that it affects the ability to drive or operate machinery, but if symptoms of dizziness, drowsiness, or confusion occur during use, driving or dangerous activities should be avoided. In cases of mild to moderate liver dysfunction, no special dosage adjustment is needed; however, efficacy may decrease in patients with severe liver dysfunction. This drug has a low potential for drug interactions, but care should be taken as blood concentrations may change when taken with certain medications. The safety during pregnancy is not certain, so women who are pregnant or may become pregnant should take this medication cautiously after consulting a physician. Breastfeeding women should also use this medication only when absolutely necessary due to the possibility of excretion into breast milk. It is recommended that children under 18 years of age do not take this drug due to insufficient confirmation of safety and efficacy. For the elderly, in general, no dosage adjustment is needed, but alterations may be required in cases of reduced kidney function. If this drug is taken in excess, immediate medical attention should be sought, and hemodialysis may be helpful. Store this medication in a dry place below 30 degrees Celsius, out of reach of children. Also, transferring to another container may lead to quality deterioration or accidents, so it should be avoided.
