Treatment of herpes zoster and treatment and suppression of recurrence of genital herpes
This medication is used to treat herpes zoster virus infections and genital herpes infections. It also has the effect of suppressing the recurrence of recurrent genital herpes. In other words, it helps to alleviate symptoms related to herpes zoster and genital herpes and prevent recurrences.
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir orally per dose, 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of an initial episode of genital herpes Adults: Administer 250 mg of this medicine orally per dose, 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer 125 mg of this medicine orally per dose, twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally per dose, twice daily. Temporarily discontinue treatment every 6~12 months to determine whether the natural course of the disease has changed. 5. Patients with renal impairment: Since creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be made with caution. The dosage for these patients is as follows. 1) Treatment of herpes zoster and an initial episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose, twice daily 10~29 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 125 mg per dose, twice daily 10~29 125 mg per dose, once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose, twice daily 10~29 125 mg per dose, twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following equations. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is preferable, taking the interval between dialysis sessions into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is required in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.
This medication should not be administered to certain patients. Patients who have had allergic reactions to this drug or its components, especially penciclovir, should not use it. Additionally, since it contains lactose, it should not be given to patients with lactose intolerance or genetic lactose metabolism disorders. It is also safer for individuals allergic to soybean oil, soy, or peanuts to avoid this medication. Side effects such as headache, dizziness, nausea, vomiting, and skin rash may occur while taking this medication, so consult a doctor if symptoms are severe. Particularly in the elderly, drowsiness or confusion symptoms may appear more frequently, so caution is necessary. This medication does not cure genital herpes; therefore, even during treatment, sexual contact should be avoided if symptoms are present, and safe sexual practices are recommended. Caution should be taken when driving or operating machinery if dizziness or drowsiness occurs. For patients with limited liver function, generally there is no need for dosage adjustment, but caution is needed in cases of severe liver impairment as therapeutic effects may decrease. While there are generally few interactions with other medications, when used in conjunction with certain drugs like probenecid, blood concentrations may increase, posing a risk of toxicity, so monitoring is necessary. Pregnant or breastfeeding women should only use this medication when the benefits outweigh the risks, as its safety has not been fully established. It is advisable not to use this medication in children under the age of 18, as safety and efficacy have not been established. Elderly patients generally do not require dosage adjustments, but adjustments may be needed if their kidney function is impaired. In case of overdose, appropriate treatment should be obtained immediately at a hospital, and hemodialysis can reduce the drug concentration. This medication should be stored in a cool, dry place below 30 degrees Celsius, in a tightly closed container, and out of reach of children. Additionally, transferring it to another container should be avoided, as it can lead to accidents or degradation of quality.
