Summary

Main use
Treatment of herpes zoster virus infection
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer famciclovir 750 mg orally once daily for 7 days. Take it at the same time each day. It is advisable to start treatment as soon as symptoms appear after infection. 2. Patients with renal impairment: Because creatinine clearance of this drug is decreased in patients with renal impairment, dose adjustment should be made with care. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose 2 times daily 10~29 250 mg per dose once daily 2) If only serum creatinine is known, creatinine clearance can be calculated using the following formulas. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 3) Patients with renal impairment undergoing hemodialysis: It is advisable to administer the drug every 48 hours, taking the dialysis interval into account. Four hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%; therefore, administer the full dose immediately after dialysis. 3. Patients with hepatic impairment: No dose adjustment is required for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This drug should not be administered to patients who have had hypersensitivity to this drug, its components, or to the pharmacologically active metabolite, penciclovir, formed in the body. Caution is required when using this drug in patients with impaired renal function, and doses may need to be adjusted or their condition closely monitored. Symptoms such as headache, nausea, vomiting, dizziness, and drowsiness may occur during treatment, and especially in elderly patients, drowsiness or confusion may occur more frequently. Rarely, there may be a decrease in platelet count or a reduction in white blood cells and neutrophils, and consultation with healthcare providers is necessary if such symptoms occur. Gastrointestinal side effects such as abdominal pain, diarrhea, liver function abnormalities, and jaundice have been reported, and skin rashes or itching, and rarely severe skin reactions may also occur. In patients with poor renal function, there is a risk of acute renal failure, so dosage should be adjusted according to renal status. This drug does not completely cure genital herpes, so it is important to avoid sexual contact and maintain safe sex practices during treatment. No special effects have been reported while driving or operating machinery; however, caution should be exercised if dizziness, drowsiness, or confusion occurs. For patients with mild to moderate liver dysfunction, no special dose adjustments are necessary, but in patients with severe liver dysfunction, careful consideration is required as drug efficacy may decrease. Interactions with other drugs are relatively few, but caution should be taken when used with drugs such as probenecid that are eliminated by the kidneys as blood levels may increase. Women who are pregnant or breastfeeding should use this drug only on the basis that the benefits outweigh the risks, as safety has not been established. Usage is not recommended for children under 18 years as safety and effectiveness have not been fully established. Generally, no dosage adjustment is needed for elderly patients; however, dosage should be adjusted if renal function is impaired. In case of overdose, appropriate supportive treatment and symptomatic treatment should be carried out, and blood dialysis may be used to lower drug concentration if necessary. This drug should be stored in a sealed container to protect it from air or moisture and kept in a cool, dry place at a temperature below 30 degrees Celsius. It should be kept out of reach of children, and transferring to another container is not advisable for maintaining quality.
