Treatment of varicella-zoster virus infection
Herpes zoster virus infection causes a rash in the form of a band on the skin and severe pain. This medication is used to alleviate symptoms caused by this viral infection, reduce inflammation, and improve pain. Therefore, it is effective for the treatment of herpes zoster.
1. Treatment of herpes zoster Adults: Administer famciclovir orally at a dose of 750 mg once daily for 7 days. Take it at the same time each day. It is preferable to start treatment as soon as symptoms appear after infection. 2. Patients with renal impairment: Because the creatinine clearance of this drug is reduced in patients with renal impairment, dose adjustments should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster Creatinine clearance (mL/min/1.73㎡) 30–59: 250 mg per dose, twice daily 10–29: 250 mg per dose, once daily 2) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 3) Patients with renal impairment undergoing hemodialysis: For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Because 4 hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%, administer the full dose of this drug immediately after dialysis. 3. Patients with hepatic impairment: No dose adjustment is required for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.
This drug should not be used in patients who have had hypersensitivity reactions to this drug or any of its ingredients, including the active substance, penciclovir. Patients with renal function problems should use this medication cautiously, and dosage adjustments may be necessary as needed. Mild headache or nausea may occur while taking this medication, but these symptoms are usually mild or moderate. There have been rare reports of decreased platelet counts and a reduction in white blood cells or neutrophils as blood-related issues. In the central nervous system, headache is very common, dizziness often occurs, and symptoms such as confusion, drowsiness, and hallucinations may rarely appear. Regarding cardiac issues, palpitations may rarely occur. In the gastrointestinal system, nausea, vomiting, abdominal pain, and diarrhea may frequently occur. Liver function test results may be abnormal, and jaundice may rarely occur. Skin reactions such as rashes or itching are common, and hives or swelling of the face, around the eyes, or in the throat may rarely occur. In patients with reduced kidney function, acute renal failure has been rarely reported. This medication does not completely cure genital herpes, so if symptoms occur during treatment, sexual contact should be avoided, and safe sexual practices should be maintained. If symptoms such as dizziness, drowsiness, or confusion occur during treatment, driving or operating machinery should be avoided. Patients with mild to moderate liver function impairment usually do not need special dosage adjustments if renal function is normal. This drug has few interactions with other drugs, but caution is needed when taken alongside drugs that affect renal excretion. Women who are pregnant or potentially pregnant should only use this drug if the benefits of treatment are judged to outweigh the risks. Women who are breastfeeding should also carefully consider the benefits and risks of using this medication. Safety and efficacy for pediatric patients under 18 years of age have not been established, so its use is not recommended. Generally, elderly patients do not need dosage adjustments, but dose adjustments should be made if there is a decline in renal function. In case of overdose, appropriate treatment is needed, and blood dialysis can reduce the level of the active ingredient of this drug. Store this medication in a dry place at or below 30 degrees Celsius, away from children and out of their reach. Avoid transferring the drug to another container as it may adversely affect quality.
