Treatment and suppression of recurrence of genital herpes; treatment of herpes zoster
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer famciclovir orally at a dose of 250 mg 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes Adults: Administer this drug orally at a dose of 250 mg 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer this drug orally at a dose of 125 mg twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this drug orally at a dose of 250 mg twice daily. Discontinue treatment periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: As creatinine clearance of this drug is reduced in patients with renal impairment, adjust the dose with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose, twice daily 10~29 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 125 mg per dose, twice daily 10~29 125 mg per dose, once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose, twice daily 10~29 125 mg per dose, twice daily 4) If only serum creatinine is known, creatinine clearance may be calculated using the following formula. Men: {weight(kg) × (140-age)}/ {72 × serum creatinine(mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, administration every 48 hours is recommended, taking the dialysis interval into account. As 4 hours of hemodialysis reduces plasma concentrations of this drug by approximately 75%, administer this drug immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This medication should not be used in patients with known hypersensitivity or allergy to specific ingredients. Additionally, as it contains lactose, it should not be administered to patients with lactose intolerance or specific genetic metabolic disorders. Caution is required when using this medication in patients with renal impairment, and dosage adjustments may be necessary. Patients may experience side effects such as headache, dizziness, nausea, vomiting, skin rash, and itching; if any symptoms occur, consultation with a healthcare professional is advised. Particularly elderly patients or those with renal impairment should be cautious as central nervous system symptoms like drowsiness or confusion may occur. This medication does not cure genital herpes, and it is important to avoid sexual contact and maintain safe sexual relations if symptoms persist during treatment. Special care should be taken when driving or operating machinery if dizziness or drowsiness occurs. While no special dosage adjustments are required for patients with mild to moderate liver impairment, there is insufficient research on patients with severe liver impairment, so caution should be exercised. This medication generally does not interact significantly with other drugs, but when used in combination with certain medications, monitoring of blood concentration changes or potential toxicity is necessary. Women who are pregnant or breastfeeding should only use this medication when the therapeutic benefits are considered to outweigh the risks, as safety has not been fully established. Use in children under 18 years of age is not recommended as safety and efficacy have not been established. In case of overdose, immediate medical attention should be sought for appropriate treatment, and blood dialysis may lower the drug concentration. Store this medication in a dry place below 30 degrees Celsius, protected from air and moisture, and keep it out of reach of children. Moreover, changing the container for storage should be avoided to maintain quality.
