As part of parenteral nutrition, this medication provides the body with the energy and essential fatty acids it needs. It is suitable for patients who are unable to take oral intake, helping to improve nutritional status and overall health recovery. Adequate supply of energy and fatty acids plays an important role in maintaining health by facilitating metabolic functions in the body. Therefore, this drug is used for parenteral nutrition.

How it is given

Adult administration

Generally, as refined soybean oil, the dosage should not exceed 2 g per day per kg of body weight (20 mL for 10%, 10 mL for 20%). The infusion time should be over 3 hours. On the first day, for the initial injection, administer up to 1 g per kg of body weight. Infuse according to the requirements for essential fatty acids and caloric intake according to the patient’s physiological metabolic state.

Precautions

This medication should be used with caution in premature or low-birth-weight infants, as it can increase serum free fatty acid levels. In particular, it can be fatal in premature infants with severe respiratory disease due to fat accumulation in pulmonary vessels. Do not use this medication in patients with thrombosis, severe liver dysfunction, severe bleeding disorders, hyperlipidemia, ketoacidosis accompanying diabetes, acute shock, electrolyte imbalance, unstable metabolic states, or chylothorax. It should also not be used in patients with allergies to phospholipids, soybean oil, beans, or peanuts. Use with caution in patients with liver dysfunction, bleeding disorders, respiratory disorders, premature infants, severe bacterial sepsis, pregnant individuals, brain disorders, acute pancreatitis, coma, or fat metabolism disorders. Long-term use of this medication may cause liver dysfunction, so liver function and serum lipid levels, along with blood tests and coagulation capabilities, should be monitored regularly. Adverse reactions such as hypotension, tachycardia, dyspnea, nausea, vomiting, diarrhea, fever, chills, facial flushing, etc., may occur, and if symptoms appear, administration should be immediately stopped and appropriate measures taken. In case of overdose, symptoms of hyperlipidemia may occur, and stopping the administration will improve symptoms. This medication may reduce the effect of warfarin, and co-administration with heparin may affect serum lipolysis, so caution is needed. For administration to newborns or premature infants, it should be infused slowly due to low fat processing capacity, and serum lipid levels should be monitored. In the elderly, physiological function may be reduced, so this medication should be administered cautiously. During blood testing while administering this medication, care must be taken as it can affect the test results until the fat is completely cleared. This medication is only administered by intravenous infusion, and rapid infusion can lead to symptoms such as phlebitis, vascular pain, nausea, or vomiting, so it should be infused slowly. Do not mix with other medications, and avoid administration of this medication for a certain period after the use of blood volume expanders. If the temperature of the infusion solution is low, it may lead to phlebitis or vascular pain, so it should be kept at an appropriate temperature. Discard any unused medication after use, and do not use medication that has been frozen once. It should be stored in a sealed container to prevent air ingress. Additionally, it is important not to freeze the medication and to store it in a cool place below 25 degrees.

Side effects and when to seek care immediately

Serious side effects

Repeated dyspnea, cyanosis, chest tightness, severe edema (including facial edema), severe allergic reactions (hypersensitivity), and coma.

Mild side effects

Occasionally, symptoms such as phlebitis, vein pain, bleeding tendency, recurrent venous thromboembolism, reticulocytosis, hypotension, tachycardia, hyperventilation, nausea, vomiting, diarrhea, thirst, fever, chills, facial flushing, facial edema, chest tightness, unpleasant odor, priapism, hyperlipidemia due to hyperlipidemic syndrome, fat deposits, prolonged blood coagulation time, and thrombocytopenia may occur.

Not everyone experiences these, and they can vary from person to person.

This medication should be stored in a sealed container to prevent air ingress. It is important not to freeze the medication and to keep it in a cool place below 25 degrees.
