Summary

Main use
Treatment of symptoms of Alzheimer-type dementia
Who takes it
Adults · Children
Adult dose
○ Adults: Administer 5 mg of donepezil hydrochloride once daily at bedtime. If sleep disturbances such as unusual dreams, nightmares, or insomnia occur, morning administration of this medicine may be considered. Since donepezil concentrations reach steady state 15 days after administration and the frequency of adverse reactions may be affected by the rate of dose escalation, administer the 5 mg dose for 4~6 weeks. After evaluating the clinical response during this period, the dose may be increased to 10 mg. When increasing the dose to 10 mg daily, administer with caution for gastrointestinal adverse reactions. When this medicine is discontinued, its effects gradually diminish; no rebound effect occurs with abrupt discontinuation. ○ Women aged 85 years or older with low body weight: Adverse reactions occur frequently, so careful monitoring is required. Elderly women with low body weight should not exceed 5 mg daily.

Precautions

This medication contains the ingredient donepezil hydrochloride and is primarily used for treating symptoms of Alzheimer's dementia. This medication should not be administered to certain patients, for instance, those who are allergic to this drug or its components, pregnant women or those who may become pregnant, and individuals with genetic lactose-related issues such as lactose intolerance or galactose intolerance. Patients with heart disease or cardiac conduction disorders should be very cautious when using this medication as it can slow the heart rate or cause arrhythmias. Those taking non-steroidal anti-inflammatory drugs or with a history of peptic ulcers should be cautious since this medication may increase gastric acid secretion and gastrointestinal motility, potentially worsening ulcer symptoms. Patients with asthma or obstructive pulmonary disease should be careful as this medication can constrict bronchial smooth muscle and increase mucus secretion, worsening symptoms. For patients with extrapyramidal disorders such as Parkinson's disease, this medication may worsen their symptoms, hence should be administered cautiously. This drug contains the dye Yellow No. 5, and those with a history of allergies to this component should also exercise caution. During treatment, various adverse reactions may occur; severe cases may lead to fainting, heart block, gastrointestinal bleeding, liver dysfunction, seizures, motor disturbances, acute pancreatitis, or acute renal failure. Milder symptoms frequently reported include nausea, vomiting, diarrhea, decreased appetite, dizziness, insomnia, headache, muscle pain, fatigue, and others. This drug may affect the cardiovascular system, potentially leading to slowed heart rate or conduction abnormalities, necessitating regular ECG monitoring for patients with heart disease. This medication is not approved for use in other forms of memory disorders apart from dementia and treatment should always be conducted under the supervision of an experienced physician. This medication may affect driving or machine operation capabilities, so caution is required during the initial dosage or when adjusting the dose. Women who are pregnant or breastfeeding are advised against using this medication, and if it must be used, consultation with a professional is required, which may involve discontinuation of breastfeeding. Safety for pediatric use has not been established, therefore it should not be used in children. This medication may interact with other drugs, especially when taken in conjunction with specific antibiotics, antidepressants, antiarrhythmic drugs, and antipsychotic agents. In cases of overdose, severe symptoms such as vomiting, bradycardia, and respiratory depression may occur, requiring immediate medical attention, and atropine may be used as an antidote. Finally, this medication should be kept out of reach of children and should not be transferred to other containers.
