Treatment of herpes zoster; treatment and suppression of herpes simplex; treatment of genital herpes and suppression of recurrence
This medication is used for the treatment of various herpes virus infections, including herpes zoster and genital herpes. Tablets ranging from 125mg to 750mg are effective for the treatment and recurrence prevention of herpes zoster virus infections and genital herpes. It can be safely used in patients with weakened immune systems, aiding in the treatment and suppression of herpes zoster and simplex herpes infections. Additionally, cream formulations are suitable for treating simplex herpes, such as cold sores. This drug is used for treating herpes virus infections and preventing recurrences.
1. Treatment of herpes zoster infection Adults: Administer 250 mg of famciclovir orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer 250 mg of this medicine orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer 125 mg of this medicine orally twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally twice daily. Discontinue treatment periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Since the clearance of this medicine is decreased in patients with renal impairment, dose adjustment should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily 10~29: 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose twice daily 10~29: 125 mg per dose once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily 10~29: 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis It is preferable to administer the medicine every 48 hours, taking the dialysis interval into account. Four hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.
This medication should not be used in certain patients. For example, patients who are allergic to this drug or its ingredients, or have a genetic disorder that makes them unable to digest lactose should not take it. Patients with renal function problems need to use this medication with caution, as dosage adjustments may be necessary. Side effects that may occur include headache, nausea, vomiting, dizziness, skin rash, among others. Most are mild to moderate, although some rare but severe skin reactions or worsening renal function have been reported. This medication does not completely cure genital herpes, so it is important to avoid sexual contact and maintain safe sex practices if symptoms persist during treatment. Treatment should be started immediately if recurrent symptoms appear. If dizziness, drowsiness, or confusion occur during use, avoid driving or operating machinery. Patients with slightly reduced liver function do not need special dosage adjustments, but those with severe liver dysfunction may experience reduced efficacy from this medication. This drug has a low risk of interactions with other medications, but care should be taken when using it together with specific medications as blood concentrations may change. There is a risk of toxicity when combined with drugs affecting renal excretion, so follow a doctor's instructions. Pregnant or breastfeeding women should carefully consider the risks and benefits of using this medication, while fertile women have no specific cautions. Children under 18 years of age should not be administered this drug as its safety and efficacy have not been established. Elderly patients may require dosage adjustments if they have renal function issues. In cases of overdose, appropriate treatment is needed, and hemodialysis may reduce the concentration of the active ingredient in this medication.
