Symptoms of acute coronary syndrome, symptoms of atherosclerosis, reduction of thrombotic risk in atrial fibrillation
This medication is used to improve symptoms caused by atherosclerosis in adult patients with ischemic stroke, myocardial infarction, and peripheral artery disease. It also helps reduce side effects related to cardiovascular issues in patients with acute coronary syndrome, especially those who have unstable angina or non-Q wave myocardial infarction and receive medical treatment, percutaneous coronary interventions, or coronary artery bypass surgery. Moreover, it is effective in reducing the risk of stroke and thromboembolism in patients with atrial fibrillation where vitamin K antagonists are not suitable and there is low bleeding risk. Therefore, this medication is widely used for the improvement of symptoms and prevention of complications in various vascular-related diseases.
○ Adults 1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), initiate treatment on the first day with a loading dose of 300 mg of this drug once daily, followed by a maintenance dose of 75 mg orally once daily. Aspirin 75 ~ 325 mg should be administered concomitantly with this drug once daily. 3. For patients with atrial fibrillation, administer 75 mg of this drug orally once daily. Aspirin 75 ~ 100 mg should be administered concomitantly with this drug once daily. This drug may be administered with or without food.
This medication may have reduced effectiveness in patients with genetically reduced CYP2C19 enzyme function due to lower levels of the active metabolite, thus these patients should reconsider treatment options or consider alternative therapies. Additionally, using this medication with drugs that activate CYP2C19 may increase the concentration of the active metabolite, raising the risk of bleeding; therefore, combination with such drugs is not recommended. This medication should not be given to patients who have hypersensitivity to it or have active bleeding, severe liver impairment, nursing mothers, or patients with genetic issues such as lactose intolerance. In patients with high bleeding risk or blood coagulation abnormalities, those who have recently undergone or are scheduled for surgery, and those on other bleeding risk medications, this medication should be used cautiously, and discontinuation of its use should be considered prior to surgery. Since this medication can induce bleeding, patients should contact a physician immediately if bleeding symptoms appear, particularly in areas with high bleeding risk such as gastrointestinal tract or eyes. Hematological adverse reactions such as neutropenia or thrombocytopenia may occur, so tests should be conducted immediately if there are signs of infection or bleeding. Gastrointestinal symptoms can include abdominal pain, dyspepsia, diarrhea, etc., and skin adverse reactions such as rashes or pruritus have also been reported. When used with other bleeding risk medications, anticoagulants, non-steroidal anti-inflammatory drugs, selective serotonin reuptake inhibitors, etc., caution is needed as the risk of bleeding may be increased. The risk to the fetus in pregnant women is not significant, but there is a risk of bleeding during delivery, so administration should be stopped a certain period prior to delivery, and nursing mothers should consider stopping breastfeeding or discontinuing the medication as it is unclear if it is secreted in breast milk. Safety and efficacy in children have not been established, and elderly patients have a higher risk of adverse reactions such as bleeding due to decreased physical function, hence dosage adjustment and careful monitoring are required. In cases of overdose, the risk of bleeding may increase, and treatment may be needed with platelet transfusions so medical attention should be sought immediately. Store in a place out of reach of children and do not transfer to another container.
