Improvement of blood sugar control in type 2 diabetes
This medication is used as an adjunct to dietary and exercise therapy to help control blood sugar in patients with type 2 diabetes. It can be administered alone and, if adequate blood sugar control is not achieved with oral hypoglycemic agents, can be combined with insulin. Furthermore, when glycemic control is difficult with sulfonylureas or metformin monotherapy, it can be used in combination with metformin to enhance blood sugar control. This medication effectively improves blood sugar levels in patients with type 2 diabetes by complementing various treatment modalities.
Administer orally once a day. Swallow whole with more than 1/2 a cup of water. It should be taken before breakfast or immediately before the first meal. The initial dose for monotherapy is glymepiride 1 mg, which is increased by 1 mg at 1 to 2-week intervals. Generally, additional effect is negligible for doses above 4 mg, although some patients may increase the dose to 6 mg or 8 mg. The maximum daily dosage is 4 mg.
This medication should not be used in patients with insulin-dependent diabetes or in those with diabetic ketoacidosis or diabetic coma. It is also contraindicated in patients allergic to this medication or sulfonylurea drugs, as well as in those with severe liver or kidney dysfunction. Women who are pregnant or may become pregnant, and those who are breastfeeding, should also refrain from using this medication. Insulin therapy is recommended for patients with severe infections or those undergoing surgery or with serious trauma, thus this medication should be avoided. Patients with gastrointestinal disorders that impair food absorption should be cautious due to the risk of hypoglycemia. Patients with genetic lactose intolerance or specific enzyme deficiencies should not take this medication. During the first week of taking this medication, the risk of hypoglycemia is heightened, so blood sugar levels should be closely monitored. Elderly patients, those with poor nutritional status or irregular eating patterns, frequent alcohol consumers, and patients with weakened kidney or liver function should be attentive. Additionally, those with thyroid disorders or other endocrine issues may also have an increased risk of hypoglycemia. When taking other medications, blood sugar changes may occur due to interactions, so consultation with a physician is necessary. This medication can induce hypoglycemia, with symptoms including headache, severe hunger, nausea, fatigue, confusion, drowsiness, and loss of consciousness. Severe hypoglycemia may present with symptoms similar to a stroke, so immediate ingestion of sugar or assistance from medical personnel is required. Also, in the initial period of taking this medication, blurred vision, gastrointestinal symptoms, liver function abnormalities, blood anomalies, and allergic reactions may rarely occur. To maintain blood sugar control, medication use must be combined with dietary management, exercise therapy, and weight control. Patients and their families should be sufficiently educated about hypoglycemia symptoms and risks, and be prepared to consume sugar immediately if hypoglycemia occurs. In situations of high stress, surgery, or infection where blood sugar control becomes difficult, transitioning to insulin therapy is advisable. This medication is excreted via the liver and kidneys; therefore, elderly patients or those with impaired kidney function should adjust the dosage, and in cases of overdose, serious hypoglycemia may occur, necessitating immediate medical attention. To store this medication safely, keep it out of reach of children and store it sealed in its original container. Lastly, women who are pregnant or breastfeeding should not take this medication without consulting a physician.
