Precautions

This medication should not be used by women who are pregnant or may become pregnant, or by breastfeeding women. It should not be administered to patients with gastrointestinal bleeding or mechanical obstruction, as it may stimulate gastrointestinal motility and worsen symptoms. It should be avoided in patients with certain tumors such as pheochromocytoma due to the risk of hypertension. It should not be given to patients who are allergic to this medication or are intolerant. It is advisable not to use this medication in patients with liver disease or in a manic phase of bipolar disorder. For patients with malignancies in the breast, this medication may increase prolactin secretion, causing breast issues, and should not be administered. Patients with lactose intolerance or genetic lactose-related disorders should not use this medication due to the lactose it contains. Caution is advised when using this medication in patients with hypertension, and care should be taken to avoid overdose in children, as extrapyramidal symptoms may occur. Elderly patients often have decreased renal function, which may lead to increased drug concentrations, so careful administration is necessary. When taking this medication, monitor for endocrine abnormalities such as amenorrhea, galactorrhea, and gynecomastia. Occasionally, extrapyramidal symptoms such as tremors or tongue twisting, restlessness, may appear, and if such symptoms occur, the medication should be discontinued immediately. Long-term use may lead to involuntary movements around the mouth, necessitating attention. Gastrointestinal issues such as dry mouth, heartburn, nausea, vomiting, diarrhea, and constipation can occur. Cardiac-related side effects such as hypertension or arrhythmias may develop, so monitoring is necessary. Rarely, serious adverse effects such as neuroleptic malignant syndrome may occur, presenting with high fever, muscle rigidity, and altered consciousness; in such cases, the medication must be stopped immediately and emergency treatment sought. Skin reactions or swelling may occur infrequently, along with symptoms such as flushing, fatigue, insomnia, drowsiness, dizziness, wobbliness, erectile dysfunction, and fluctuations in libido. This medication may increase prolactin secretion, which can lead to side effects such as milk leakage; patients should be informed in advance. Caution should be observed regarding the potential for extrapyramidal symptoms and endocrine dysfunction. It is advisable to avoid tasks requiring concentration, such as driving or operating machinery, while taking this medication. Special caution should be exercised in patients with heart disease, especially when taking high doses, due to the risk of cardiac arrhythmias. This medication may interact with other drugs, so special care is needed when used with anticholinergics, opioid analgesics, digitalis preparations, benzamides, psychiatric medications, drugs that prolong the QT interval, and medications that cause electrolyte disturbances. It should be avoided in combination with alcohol. It should not be used by pregnant or breastfeeding women due to the lack of established safety for the fetus or newborn. Elderly patients should adjust the dose and dosing intervals due to prolonged retention of the medication in the body caused by decreased renal function, while being cautious of side effects. This medication has anti-nausea effects, which can mask or exacerbate vomiting symptoms caused by other conditions; therefore, the patient's condition should be carefully monitored. Animal studies have reported the possibility of reproductive harm and tumor formation, so caution is advised with long-term use.
