This medication is used for the treatment and prevention of herpes simplex virus infections that occur on the skin and mucous membranes, including primary and recurrent genital herpes. It is also effective in the treatment of herpes zoster virus infections and particularly helps alleviate acute pain. However, please note that its effectiveness against postherpetic neuralgia has not yet been conclusively proven. Additionally, it is used to treat chickenpox in children over two years of age. Overall, this drug is useful for treating various viral infections and improving symptoms.

Dosage and Administration

Adult dosage

1. Precautions

This medication should not be used in patients who have had allergic reactions to this drug or its components, as well as to valacyclovir. Caution is needed when administering to patients with impaired kidney function, as the likelihood of central nervous system adverse reactions may be increased. Patients with liver impairment should use this drug with caution as it may further worsen liver function. In infants and children, it should be administered minimally only when necessary, and care should be taken with neonates or low birth weight infants, as safety has not been established. Elderly patients may have diminished renal function, which could lead to increased blood concentrations of this drug and a higher risk of adverse reactions, requiring careful adjustment of dosage and intervals. During administration of this drug, rare but serious allergic reactions such as anaphylactic shock may occur, so patients should be closely monitored. If symptoms such as hypotension, difficulty breathing, or chest tightness occur, administration should be discontinued immediately and appropriate treatment should be sought. Photosensitivity reactions or hypersensitive reactions such as skin rashes, fever, vesicles, and urticaria may occur; if these symptoms are observed, administration should be stopped. Skin-related adverse reactions such as hair loss, erythema multiforme, severe skin conditions such as Stevens-Johnson syndrome or Lyell's syndrome may occur, thus caution is warranted. Neurological symptoms may include consciousness disturbances, spatial disorientation, impaired concentration, aggressive behavior, confusion, hallucinations, seizures, drowsiness, anxiety, etc., especially in the elderly or patients with impaired renal function, where these may occur more frequently. Renal issues such as renal failure, hematuria, and proteinuria may develop; hence, kidney status should be carefully observed. Hematologic and lymphatic abnormalities may present as leukopenia, anemia, bleeding, or purpura, so regular blood tests are advisable. Liver function abnormalities may manifest as hepatitis, jaundice, or elevated liver function tests, requiring careful monitoring of liver status. Gastrointestinal symptoms such as gastritis, nausea, vomiting, diarrhea, and abdominal pain may occur; should symptoms develop, administration should be reconsidered. Systemic symptoms may include fever, swelling, muscle weakness, and headache. Musculoskeletal issues may involve joint pain and myalgia. Cardiovascular problems may present as arrhythmias, blood pressure changes, and palpitations, therefore cardiac status should be monitored. Urinary issues may include difficulty urinating or urinary obstruction. This drug is effective when administered soon after the onset of symptoms during herpes simplex or chickenpox treatment; for shingles, it should be started within 5 days after skin symptoms appear, and for chickenpox, within 3 days is preferable. Patients with impaired immune function might need intravenous formulations. Adequate hydration is necessary during the treatment period to reduce risks of dehydration and renal toxicity. In treating recurrent genital herpes, it should only be used in patients with repeat outbreaks, and the decision to continue treatment should be evaluated after one year. This medication may cause consciousness disturbances, hence caution is required during driving or operating hazardous machinery. Interactions may occur when taken with other drugs, especially those affecting kidney function or specific medications, increasing the risk of renal impairment, hence caution is advised. Women who are pregnant or may become pregnant should use this medication only when the benefits outweigh the risks, and since the components of this drug are found in breast milk, breastfeeding should be discontinued while on treatment. In cases of overdose, serious side effects such as agitation, stupor, seizures, and renal failure may occur, necessitating immediate medical observation and appropriate treatment.
