Precautions

This medication should not be used in patients who have had hypersensitivity reactions to this medication or its components. In patients with gastrointestinal blockage at the pylorus, duodenum, or intestine, this medication should not be administered as it may exacerbate symptoms by inhibiting the motility of the gastric smooth muscles. Patients with gastroparesis or intestinal atony should use caution as the anticholinergic effects of this medication may worsen symptoms. In patients with urinary retention, this medication should not be administered as it can hinder bladder contractions and worsen symptoms. Patients with angle-closure glaucoma should not use this medication due to the risk of elevated intraocular pressure from its anticholinergic effects, which can worsen glaucoma symptoms. Patients with severe myasthenia gravis should avoid this medication as its anticholinergic action may worsen muscle weakness symptoms. Patients with severe heart disease should exercise caution as this medication may lead to arrhythmias or extrasystoles. Patients with toxic megacolon or ulcerative colitis should not use this medication as there is a risk of exacerbating symptoms. Patients with moderate to severe liver function impairment should use this medication cautiously due to the risk of adverse reactions since it is metabolized in the liver. Patients with a history of arrhythmia or current arrhythmias should exercise caution as this medication may worsen heart rhythm problems. This medication contains lactose; therefore, patients with lactose intolerance or related genetic disorders should not take it. Patients with urinary difficulties should use this medication cautiously as it may exacerbate symptoms or increase post-void residual volume. Patients with liver or kidney function problems or a history of such conditions should monitor the risk of adverse reactions due to the potential impact on the metabolism and excretion of this medication. Patients with Parkinson's disease or cerebrovascular disorders should be administered this medication with caution as it may worsen symptoms or result in neurological side effects. Patients with hyperthyroidism may experience exacerbation of sympathetic nervous system symptoms, such as tachycardia, due to the anticholinergic effects of this medication. Elderly patients may have decreased liver and kidney function; therefore, it is important to start with a lower dose and administer cautiously. Patients with specific diseases such as autonomic neuropathy, coronary artery disease, severe congestive heart failure, tachycardia, benign prostatic hyperplasia, cerebral sclerosis, and gastroesophageal reflux disease should also use this medication cautiously. Patients with glaucoma should be careful as this medication may raise intraocular pressure beyond angle-closure glaucoma and worsen symptoms due to its anticholinergic effects. While taking this medication, symptoms of hallucinations or delusions as mental neurological disturbances may appear; thus, it is important to observe for such symptoms and consult a physician immediately if they occur. Severe constipation or abdominal distension indicating a paralytic ileus may rarely occur in the digestive system; therefore, any discomfort during the administration should be reported to the healthcare provider. Cardiac-related adverse reactions such as palpitations, irregular heartbeats, bradycardia, and ventricular tachycardia have been reported, and symptoms should be monitored closely. Blood-related side effects such as thrombocytopenia may occur; hence, patients should report any unusual symptoms to healthcare providers immediately. Kidney function impairment or urinary retention may occur; therefore, kidney function should be monitored regularly, and attention should be paid to any unusual symptoms. Skin reactions, such as rashes, itching, or in severe cases, serious skin reactions like Stevens-Johnson syndrome, may occur; thus, close observation of the skin condition is essential. Acute glaucoma attacks due to elevated intraocular pressure may rarely occur; thus, immediate medical attention is required if symptoms like visual decline, eye pain, or headaches arise. Symptoms related to liver dysfunction may also occur; therefore, regular liver function tests are recommended. Musculoskeletal abnormalities such as myalgia, weakness, and rhabdomyolysis may occur, so it is crucial to report any unusual sensations in the muscles immediately. While taking this medication, side effects such as dry mouth, constipation, dizziness, drowsiness, and insomnia may appear, which could affect daily life, so caution is advised. This medication may increase the side effects when taken with other anticholinergics, tricyclic antidepressants, phenothiazine drugs, or MAO inhibitors; hence, patients should inform their physician or pharmacist before combination therapy. Women who are pregnant or may become pregnant should avoid taking this medication as its safety has not been established, and breastfeeding women are also advised to discontinue nursing. The safety in children has not been established; therefore, it is advisable not to administer this medication to pediatric patients. In overdose cases, severe symptoms such as delusions, excitement, convulsions, gait and speech disorders, paralytic ileus, urinary retention, cardiac irregularities, and elevated blood pressure may occur; thus, immediate emergency treatment is necessary if suspected. This medication should be kept out of the reach of children, and transferring it to other containers is not advisable for quality maintenance and safety. During the administration of this medication, it is important to confirm that clinical symptoms are clear and to ascertain the presence of other conditions showing similar symptoms; driving or operating dangerous machinery should be avoided as drowsiness or dizziness may occur.
