Summary

Main use
Treatment of herpes zoster and genital herpes and treatment and suppression of herpes simplex
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer famciclovir 250 mg orally 3 times daily for 7 days. It is advisable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer this drug 250 mg orally 3 times daily for 5 days. It is advisable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer this drug 125 mg orally twice daily for 5 days. It is advisable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this drug 250 mg orally twice daily. Temporarily discontinue treatment periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Because creatinine clearance of this drug is reduced in patients with renal impairment, dose adjustment should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose twice daily 10~29 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59 125 mg per dose twice daily 10~29 125 mg per dose once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose twice daily 10~29 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is advisable, taking the dialysis interval into account. Because 4 hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%, administer this drug immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This medication should not be used in individuals who are allergic to this medication or its components, or to its active ingredient, penciclovir. Additionally, this medication contains lactose, so it should not be administered to patients with genetic disorders such as lactose intolerance, lactase deficiency, or glucose-galactose malabsorption. Patients with impaired renal function must use this medication cautiously, and dose adjustments may be necessary. While taking this medication, side effects such as headache, nausea, dizziness, drowsiness, skin rash, itching, abdominal pain, and diarrhea may occur, so any abnormal symptoms should be discussed with a healthcare professional. Especially in elderly patients or those with renal disease, central nervous system symptoms such as drowsiness or confusion may occur more frequently and require caution. This medication does not completely cure genital herpes, so it is important to avoid sexual contact during symptoms and maintain safe sexual relations. Caution is necessary when driving or operating machinery if symptoms of dizziness or drowsiness occur. Patients with mildly impaired liver function do not generally require dose adjustments, but severe liver dysfunction should be monitored as it may decrease the drug's efficacy. As interactions with other medications may occur, especially when used concurrently with probenecid or lamotrigine, side effects or changes in drug efficacy should be closely monitored. Women who are pregnant or breastfeeding should only use this medication after consulting a physician, as its safety has not been established. Children under 18 years of age should generally avoid this medication, as its safety and efficacy have not been confirmed. Elderly patients typically do not require dose adjustments, but in cases of decreased renal function, dose adjustments may be necessary. In the event of overdose, immediately seek medical help, and blood dialysis may be beneficial. This medication should be stored in a cool, dry place below 30 degrees Celsius, out of reach of children, and transferring it to another container should be avoided.
