Improvement of blood sugar control in type 2 diabetes
This medication is used to assist dietary therapy and exercise therapy for improving blood sugar control in patients with type 2 diabetes. It can be administered as monotherapy, and when blood sugar control is inadequate with oral hypoglycemic agents, it is used in combination with insulin. Additionally, if blood sugar control is not adequate with monotherapy using sulfonylureas or metformin, it is administered in combination with metformin. This medication effectively improves blood sugar control, aiding in diabetes management.
In patients who have not received prior medication, initiate treatment with 1 mg of Glimepiride once daily. If needed, increase the dose by 1 mg at intervals of 1 to 2 weeks. Generally, there is little additional effect from doses exceeding 4 mg, but some patients have achieved better metabolic control by increasing the dose to 6 mg (or 8 mg). This medication is not to be used in patients with insulin-dependent diabetes, diabetic ketoacidosis, or coma. It should not be taken by individuals allergic to this medication, its components, or sulfonylureas. There is no experience with this medication in patients with severe liver or kidney impairment, therefore, switching to insulin therapy is recommended. Women who are pregnant or may become pregnant, as well as nursing mothers, should not take this medication for safety. Patients with severe infections, undergoing surgery, or suffering severe trauma may require insulin therapy. Caution is advised for patients with gastrointestinal disorders like diarrhea or vomiting, as food absorption may be impaired, increasing the risk of hypoglycemia. This medication contains lactose and should not be given to patients with lactose intolerance or certain genetic problems. During the first week of treatment, the risk of hypoglycemia is high, so blood sugar levels should be monitored closely. Especially in elderly patients, those with poor nutritional status or skipping meals, and patients with irregular eating habits, the risk of hypoglycemia is significant. The risk of hypoglycemia may also increase if there is an imbalance between exercise and carbohydrate intake or if eating patterns change. Patients consuming alcohol, particularly those who skip meals, should be cautious of hypoglycemia. Patients with renal or hepatic dysfunction may be more sensitive to this medication, requiring dose adjustment. The risk of hypoglycemia increases if other medications are taken or overdosed. Endocrine disorders such as thyroid abnormalities, pituitary dysfunction, or adrenal insufficiency can also affect blood sugar control. This medication may cause hypoglycemia, with symptoms including headache, severe hunger, nausea, fatigue, anxiety, confusion, dizziness, drowsiness, decreased consciousness, etc. Severe hypoglycemia can lead to coma or seizures, necessitating immediate treatment. Additionally, transient visual blurring or visual disturbances may occur due to blood sugar changes at the initial dose. Gastrointestinal symptoms such as nausea, vomiting, abdominal pain, and diarrhea may occur infrequently. Abnormal liver function or blood reactions have also been reported; if these symptoms occur, immediate consultation with a doctor is essential. Allergic reactions such as hives, rash, or difficulty breathing can occur; if these symptoms appear, discontinue use immediately and inform healthcare personnel. When taking this medication, maintaining a regular meal and exercise routine, as well as weight management, is crucial; if symptoms of poor blood sugar control (e.g., frequent urination, thirst, dry skin) appear, consult your doctor. In the event of hypoglycemia, it is important to rapidly ingest sugar, which should be in the form of sugar or sugary drinks, as artificial sweeteners are ineffective. Hypoglycemia may recur during treatment, necessitating continuous observation and management. Inform other healthcare personnel about diabetes treatment, and in stressful situations due to surgery or infections, insulin therapy may be necessary. This medication may interact with other drugs, so it is important to inform healthcare personnel of any other medications being taken, as adjustments may be needed. Caution should be exercised when taken with certain antifungals, antibiotics, antihypertensives, anticoagulants, and beta-blockers. Beta-blockers and similar medications may conceal the symptoms of hypoglycemia, requiring additional caution. Women who are pregnant or breastfeeding should avoid taking this medication, and switching to insulin therapy is recommended if necessary. It is not used in children as safety and efficacy have not been established. Elderly patients should carefully adjust doses as renal function may decline. In cases of overdose, severe hypoglycemia may occur, requiring immediate emergency treatment and receiving appropriate care such as intravenous glucose in a hospital.
