This medication is used to assist in controlling blood sugar levels in patients with type 2 diabetes. It is administered as an adjunct to dietary and exercise therapy, and can also be used alone. If blood sugar control is insufficient, it may be combined with oral hypoglycemic agents or insulin. Particularly, it is used in conjunction with metformin when treatment with sulfonylureas or metformin alone is ineffective. It helps improve blood sugar control when used with various treatment methods.

How to take

Adult dose

This medication is taken orally once a day. It should be taken before breakfast or prior to the first meal, and swallowed whole with more than 1/2 cup of water. The initial dose is started at 1 mg of glymepiride, and may be increased by 1 mg every 1 to 2 weeks. Generally, there is little additional effect when a dose of more than 4 mg is administered, but some patients may require an increase up to 6 mg or 8 mg.

Precautions

This medication should not be used in patients with insulin-dependent diabetes, diabetic ketoacidosis, or in a coma. It is also contraindicated for patients with an allergy to this medication or its components, or those with severe liver or kidney dysfunction. It is safest not to use this medication in pregnant or nursing women. Patients with severe infections, before and after surgery, or those who have experienced severe trauma are recommended to use insulin therapy and should avoid this medication. Caution is advised as gastrointestinal disturbances such as vomiting or diarrhea can lead to poor absorption of food, increasing the risk of hypoglycemia. During the first week of dosing, the risk of hypoglycemia is high, so blood sugar levels should be checked frequently and monitored closely. Particularly, elderly patients, those with poor nutritional status or irregular eating habits, patients who consume alcohol frequently, and those with weakened liver or kidney function should use this medication more cautiously. This medication can cause hypoglycemia, so patients should be well aware of hypoglycemic symptoms and should consume sugar immediately when they occur. Hypoglycemic symptoms may include headache, severe hunger, dizziness, confusion, drowsiness, decreased consciousness, etc. In severe cases, loss of consciousness or coma may occur, necessitating immediate medical assistance. This medication can have various side effects, including liver dysfunction, blood disorders, and allergic reactions; therefore, if abnormal symptoms occur, consult a physician immediately. To maintain blood sugar control, regular administration of this medication along with dietary plans, exercise therapy, and weight management is important. When taking other medications, interactions may affect blood sugar control, so be sure to inform medical personnel. Caution is necessary when taken with certain antibiotics, anticoagulants, beta-blockers, steroids, etc. Overdosing on this medication can cause severe hypoglycemia, so if overdosed, immediate hospital attention and appropriate treatment are required. The safety and efficacy of this medication have not been established in children, and dosage adjustment may be necessary in elderly patients due to decreased kidney function. Keep the medication out of reach of children and store it in its original container.

Serious side effects

Urticaria, severe swelling (edema), difficulty breathing, hypotension, shock, anaphylaxis, angioedema, skin and mucosal reactions (Stevens-Johnson syndrome), liver dysfunction (cholestasis, jaundice), hepatitis, severe blood changes (thrombocytopenia, leukopenia, hemolytic anemia, erythrocytopenia, agranulocytosis, pancytopenia, aplastic anemia), severe hypoglycemia (loss of consciousness, coma, brain convulsions), severe allergic reactions.

Mild side effects

Headache, severe hunger, nausea, vomiting, fatigue, sleep disturbances, anxiety, aggression, decreased concentration, lack of agility or initiative, depression, confusion, speech disturbances, aphasia, visual disturbances, tremors, partial paralysis, sensory disturbances, dizziness, lethargy, lack of self-control, temporary mental confusion, drowsiness, bradycardia, sweating, cool and moist skin, tachycardia, hypertension, palpitations, angina, deep vein thrombosis, blurry vision, or feeling of fullness or pressure in the upper abdomen.

Not everyone experiences these, and they can vary from person to person.

Storage

This medication should be stored in a tightly sealed container to prevent exposure to air or moisture. The recommended storage temperature is between 1°C and 30°C room temperature.
