  • Blood sugar control in patients with type 2 diabetes
View approved original text

This medication is used in conjunction with diet and exercise regimens to help manage blood sugar control in patients with type 2 diabetes. It can be used alone or in combination with oral hypoglycemic agents or insulin. In particular, it may be used in conjunction with sulfonylureas or metformin when blood sugar control is difficult. These treatment methods enable effective management of blood sugar and assist in diabetes management.

How to take

Adult dose

Dosing should be individualized for each patient. Take with more than 1/2 cup of water without chewing. This medication is to be administered orally once daily, to be taken before breakfast or just prior to the first meal. The initial dosage for monotherapy should begin at 1 mg of glimepiride once daily, and may be increased by 1 mg at 1-2 week intervals. Generally, there is little additional effect at doses over 4 mg, but in some patients, improved metabolic control may be achieved by increasing to 6 mg.

Precautions

This medication should not be used in patients with insulin-dependent diabetes, diabetic ketoacidosis, or patients in a coma. Additionally, patients with allergies to this drug, its ingredients, or sulfonylureas should not receive it. There is no experience with this medication in patients with severe liver or renal impairment, thus transitioning to insulin therapy is recommended. Women who are pregnant or may become pregnant, as well as breastfeeding women, are advised against taking this medication. Patients with severe infections, preoperative or postoperative conditions, or severe trauma should also receive insulin therapy. Patients with gastrointestinal disturbances such as diarrhea or vomiting should be cautious due to the risk of hypoglycemia from impaired food absorption. This medication contains lactose, thus, patients with lactose intolerance or certain genetic metabolic disorders should avoid it. During the first week of taking this medication, there is a higher risk of hypoglycemia, so blood sugar levels should be closely monitored. Older adults, those with poor nutritional status, individuals who skip meals, and those with irregular eating habits are more susceptible to hypoglycemia. If there is an imbalance between muscle activity and carbohydrate intake, the risk of hypoglycemia also increases. Patients who frequently consume alcohol or those with impaired kidney or liver function, as well as patients with other endocrine disorders, should also exercise caution. If this medication is overdosed or taken with other drugs, blood sugar control may be affected, so caution is advised. If hypoglycemic symptoms appear, it is important to immediately consume sugars, and if symptoms are severe, consult a medical professional. This medication can cause hypoglycemia, with symptoms that may include headaches, severe hunger, nausea, fatigue, confusion, drowsiness, and in more severe cases loss of consciousness. Temporary blurred vision or indigestion, liver function abnormalities, and rare blood abnormalities and allergic reactions may also occur. If allergic symptoms such as hives, itching, or difficulty breathing occur, immediate medical attention should be sought. If severe adverse reactions occur, discontinue use and seek prompt medical treatment. While taking this medication, concurrent dietary and exercise regimens are important for blood sugar control, and it is crucial to take it regularly. In case of hypoglycemia, sugar or sugary drinks should be consumed immediately, as artificial sweeteners are ineffective. Additionally, during stressful situations, surgery, or infection, where blood sugar control is difficult, temporary transition to insulin therapy may be necessary. If taking other medications or starting new ones, it is essential to inform the physician or pharmacist about potential interactions with this medication. Special attention is required when taking it with certain antibiotics, antifungals, beta-blockers, steroids, and anticoagulants, as blood sugar changes may occur. Pregnant or breastfeeding women are recommended to switch to insulin therapy, as this medication may pose risks to the fetus or newborn. The safety and efficacy of this medication in children under 18 years of age have not been established, thus its use in this population is discouraged. In older adults, caution should be exercised regarding dosage adjustments due to potential renal function decline. In the event of an overdose, severe hypoglycemia may occur, and immediate medical treatment should be sought. When storing this medication, it should be kept in a tightly closed container, out of reach of children, and in its original container to maintain its quality and safety.
