Relief of severe pain requiring opioid analgesics
This injection is used to alleviate severe pain that requires narcotic analgesics. It effectively reduces the intense pain that patients experience, helping them achieve a state of comfort.
(Injection) * 1, 2, 4 mg/mL The initial dose is 1 ∼ 2 mg administered subcutaneously or intramuscularly every 4 ∼ 6 hours, depending on the severity of pain. The dose should be adjusted appropriately according to the severity of pain, the patient's underlying conditions, age, and body size. Patients with terminal cancer may have strong tolerance to opioid analgesics and may require higher doses for adequate pain relief. Intravenous administration may be used if necessary; however, the injection should be given slowly over at least 2 ∼ 3 minutes. Rapid intravenous administration of opioid analgesics may increase the risk of hypotension and respiratory depression. If tolerance develops or the pain worsens, gradual dose escalation may be necessary. Generally, the first sign of tolerance is a reduction in the duration of the drug's effect.
This medication is used in injection form and requires careful cutting of the glass ampoule during use. If glass fragments are mixed in, side effects may occur, especially when administered to children or the elderly. Taking this drug with central nervous system depressants such as narcotics, benzodiazepines, and alcohol can cause severe respiratory depression or coma, so co-administration should be avoided, or it should be used at the minimum dose for the shortest duration when necessary. This drug should not be administered to certain patient groups. For example, to those with allergies to this drug or its ingredients, patients with increased intracranial pressure, severe respiratory depression, heart failure due to chronic lung disease, asthma attack patients, severe central nervous system depressants, patients with acute abdominal disease, patients taking MAO inhibitors, patients in a convulsive state, patients with acute alcohol poisoning, pregnant women or women of childbearing potential, patients with liver dysfunction, etc. Additionally, caution is required in administering this drug to elderly or weakened patients, patients with kidney dysfunction, hypothyroid or adrenal insufficiency patients, central nervous system depressants, patients with toxic psychosis, patients with urinary disorders, patients with pancreatitis or gallbladder disease, patients in shock, patients with a history of seizures, patients with a history of drug dependence or alcoholism, etc. This drug can cause dependence and careful observation for signs of drug dependence is necessary with prolonged use. It can also cause respiratory depression by acting on the central respiratory center in the brainstem, requiring close monitoring of respiratory status and appropriate treatment if necessary. Because this drug suppresses the cough reflex, caution is needed for patients with lung diseases, and it may affect mental and physical abilities, requiring caution when driving or operating dangerous machinery. In case of overdose, severe respiratory depression, loss of consciousness, and hypotension may occur, necessitating immediate emergency treatment, including administration of opioid antagonists and respiratory support. Pregnant or nursing mothers should not use this medication, particularly during childbirth or immediately before and after, as respiratory depression or withdrawal symptoms may occur in newborns. The safety and efficacy of this drug have not been established in children, and elderly patients should be administered cautiously starting at lower doses. Finally, when administering this medication via intravenous injection, it should be given slowly, as rapid administration may increase the risk of side effects such as hypotension or respiratory depression.
