This medication is used to prepare dialysis solutions for patients with acute and chronic renal failure who are receiving bicarbonate-based dialysis. It is also utilized for preparing dialysis solutions required for detoxifying patients who are poisoned by toxic substances. Therefore, it is used in the process of dialysis treatment for renal failure and poisoning.

How it is given

Adult administration

1. When using a specified acetate-containing dialysis solution (Solution A), this formulation can prepare about 180L of bicarbonate-based dialysis solution containing 38mEq/L of HCO3-. 2. In cases where acetate-containing dialysis solution (Solution A) is used, this preparation should be used as dialysis solution with a dilution ratio of 1 : (this medication + purified water) 34 in the supply device for bicarbonate-based dialysis. 3. For each patient, a single-use dialysis solution (based on 6 hours) is prepared.

Precautions

This medication should not be used in patients with very low blood pressure or tachycardia. Caution is also advised in patients with heart problems or pulmonary edema. Patients with elevated potassium levels and those with coagulation disorders should avoid using this medication. It should be used cautiously in patients with severely impaired liver function or metabolic issues due to diabetes. Particularly, patients taking medications that affect the heart should be aware that this medication can lower potassium levels, potentially leading to side effects. Patients with elevated calcium and phosphate levels before dialysis should also be careful when using this medication. Those with inadequate calorie intake or dietary restrictions due to diabetes are at risk of hypoglycemia and should be cautious. As this medication may cause hypoglycemic symptoms, blood sugar should be monitored closely, and glucose injections or sugar intake should be used as needed. It may increase blood calcium levels; thus, it is recommended to use dialysis solutions with low calcium concentrations. There is a risk of complications such as osteoporosis or weakened bones, requiring appropriate treatment such as vitamin D3 supplementation. Additionally, abnormal calcium deposits in the body may necessitate treatment to control phosphate levels. If symptoms such as headache, nausea, vomiting, seizures, or altered consciousness occur during dialysis, measures like reducing the dialysis speed should be taken. There can be a rapid loss of blood volume leading to hypotension or shock, necessitating the cessation or adjustment of dialysis along with fluid and blood pressure-raising agents. Conversely, blood pressure may rarely increase, and antihypertensive medications or adjustments to the dialysis solution may be required. This medication is used in dialysis for patients with chronic renal failure, especially when metabolic acidosis is difficult to improve or when dialysis management is challenging due to hypotension. It is suitable for diabetic patients who have difficulty controlling blood sugar as it does not contain glucose. Regular blood tests to monitor electrolytes, acid-base balance, and blood sugar levels are essential. Women who are pregnant or may become pregnant should only use this medication when the therapeutic benefits outweigh the risks, as its safety has not been established. Its safety in pediatrics is also not established, so it should be used cautiously. The elderly should understand the medication's characteristics well and, if used long-term, require regular examinations and vitamin D3 supplementation to prevent bone-related complications. Dialysis water must be thoroughly purified and attention should be paid to any risk of dialysis membrane damage and bleeding. During preparation, this product should not be mixed directly with electrolyte solutions for artificial renal dialysis but rather a specified dilution should be used. Care must be taken if the concentration is too high or low, as symptoms like headache, palpitations, abnormal blood pressure, or limb numbness may occur. It is advisable to check the electrolyte concentrations and pH of the dialysis solution before use. This medication should not be used for intravenous injection or peritoneal dialysis, and the dialysis solution should be used immediately after preparation. It should be maintained at body temperature, and if stored for more than 2 hours, insoluble particles may develop and should not be used.

Hypoglycemia, hypercalcemia, osteoporosis, osteomalacia, fibrous dysplasia, ectopic calcification, dialysis disequilibrium syndrome (headache, nausea, vomiting, convulsions, altered consciousness, discomfort, fatigue), hypotension, shock symptoms, blood pressure elevation, headache, palpitations, limb numbness, generalized fatigue, chest discomfort, altered consciousness
