Administer 2.5 µg/min of dopamine hydrochloride per kg of body weight via intravenous injection. In severe cases, the dosage can be increased to 5-10 µg/min to 20-50 µg/min per kg of body weight for intravenous injection. Adjust according to age and symptoms. Before administration, infuse blood or plasma to maintain venous pressure between 10-15 cmH2O and pulmonary wedge pressure between 14-18 mmHg. If administering over 50 µg/min per kg of body weight, frequent urine volume testing should be conducted.

This medication is used for the treatment of shock, oliguria, anuria, and hypotension due to myocardial infarction, trauma, endotoxin shock, postoperative kidney impairment, and chronic heart failure (congestive heart failure) resulting in reduced cardiac output.

How it is given

Adult administration

Administer 2.5 µg/min of dopamine hydrochloride per kg of body weight via intravenous injection. In severe cases, the dosage can be increased to 5-10 µg/min to 20-50 µg/min per kg of body weight for intravenous injection. Adjust according to age and symptoms. Before administration, infuse blood or plasma to maintain venous pressure between 10-15 cmH2O and pulmonary wedge pressure between 14-18 mmHg. If administering over 50 µg/min per kg of body weight, frequent urine volume testing should be conducted.

Precautions

This medication should be used at a reduced dosage for patients who have taken monoamine oxidase inhibitors. It is not recommended for use in children as safety and appropriate dosing have not been sufficiently established. Mixing with alkaline solutions reduces the drug's efficacy; do not use any alkaline diluents other than 5% glucose injection. It should not be used in patients with specific diseases such as pheochromocytoma, uncontrolled arrhythmia, hyperthyroidism, or closed-angle glaucoma. Allergic patients should be cautious, especially those sensitive to sulfites, as asthma attacks may occur. If the drug leaks into surrounding tissues, it can cause tissue damage, so it should be injected into a large vein and the injection site should be frequently monitored. Cardiac and vascular adverse reactions may occur; thus, the patient's cardiac status should be carefully monitored. In cases of overdose, blood pressure may rise sharply, so the infusion rate should be reduced or stopped immediately if symptoms occur. During pregnancy, considering the potential risks to the fetus, it should only be used when absolutely necessary, and during breastfeeding, it is advisable to avoid administration or stop breastfeeding. In elderly patients, as liver, kidney, and heart function are often impaired, start with a lower dose and adjust it cautiously. This medication may interact with other drugs, so be sure to consult with healthcare professionals regarding any concurrently taken medications. During prolonged treatment, monitor body fluid balance and electrolyte status regularly to check for abnormalities. Before injection, check the color and turbidity of the solution; do not use it if there are any abnormalities. Sudden discontinuation may cause hypotension, so taper off gradually. This drug is sensitive to heat and light; therefore, care must be taken during storage, and it should not be mixed with alkaline solutions.

Side effects and when to seek care immediately

Serious side effects

Breathlessness, asthma attacks, risk of peripheral vascular ischemia and necrosis, hypertension, ventricular arrhythmias, pulmonary edema, severe hypokalemia, necrosis and scabbing of surrounding tissue due to extravasation, severe hypertension.

Mild side effects

Ventricular extrasystole, atrial fibrillation, ventricular tachycardia, peripheral vascular constriction resulting in changes in color and temperature of limbs, postural tachycardia, tachycardia, angina, palpitations, hypotension, hypertension, extrasystole, angina pain, vascular constriction, abnormal conduction, bradycardia, prolonged QRS width, hypernitrogenemia, increased blood pressure, nausea, vomiting, abdominal distension, abdominal pain, paralytic ileus, hair loss, headache, anxiety, phlebitis, injection site necrosis, hair growth.
