Nutritional improvement through the supply of energy and essential fatty acids.
This medication is used as part of a parenteral nutrition regimen to provide patients with the necessary energy and essential fatty acids. This helps improve the patient's nutritional status and normalize bodily functions.
Generally, up to 2 g of refined soybean oil per day per kg of body weight. Administered as an intravenous infusion over a period of more than 3 hours. On the first day, for the initial injection, up to 1 g per kg of body weight should be administered. Infusion should be adjusted according to the patient's physiological metabolic status to meet the demands for essential fatty acids and calories.
Caution is necessary as this drug can increase free fatty acid levels in the blood of premature infants and low birth weight infants. Care should be taken when administering to premature infants with severe respiratory disorders as fat accumulation in the lungs could lead to serious problems. This drug should not be administered to patients with thrombosis, severe liver dysfunction, blood coagulation disorders, hyperlipidemia, diabetic ketoacidosis, acute shock, electrolyte imbalance, unstable metabolic status, or fat overload syndrome. Patients with allergic reactions or hypersensitivity to egg, soy, or peanut proteins should also avoid using this drug. It should be administered with caution in patients with liver dysfunction, coagulation disorders, respiratory disorders, premature infants, severe bacterial sepsis, pregnant women, brain diseases, acute pancreatitis, coma, and fat metabolism abnormalities. Long-term use of this drug may lead to liver dysfunction; thus, regular liver function tests and serum lipid level monitoring are required. Various side effects such as phlebitis, bleeding tendency, hypotension, respiratory distress, nausea, vomiting, and fever may occur during administration; if symptoms appear, treatment should be stopped immediately and appropriate care should be provided. Overdose may lead to fat overload syndrome, which can result in hyperlipidemia, fever, organ abnormalities, blood coagulation delays, and in severe cases, coma. This drug contains soybean oil and phospholipids, which can rarely cause allergic reactions; careful monitoring for cross-sensitivity to soy and peanuts is needed. When used with warfarin, the effectiveness of warfarin may be reduced, and when administered with heparin, fat-decomposing enzymes may be temporarily depleted, delaying lipid clearance. When administering to newborns and premature infants, infusion should be gradual and serum lipid levels monitored due to lower fat processing capability. Newborns with respiratory distress should be closely monitored, and children suspected of having hyperbilirubinemia or pulmonary hypertension should undergo regular platelet and liver enzyme testing. Long-term administration may cause thrombocytopenia in children, requiring caution. This drug may produce degradation products when exposed to light, increasing the risk of adverse effects; therefore, it should be stored and used in a way that protects it from light especially when administered to newborns and infants under two years old. Elderly patients, who have decreased physiological functions, should be administered this drug with caution. Blood tests done shortly after administration may yield affected results, so timing should be adjusted accordingly. This drug should only be administered via intravenous infusion, and rapid infusion may lead to acute symptoms such as phlebitis, venous pain, nausea, and vomiting; thus, it should be infused slowly. It must not be mixed with other medications, and there should be a delay in administration after blood volume expanders. If the solution is at a low temperature, phlebitis and venous pain may occur, so it should be maintained at an appropriate temperature. Once opened, the solution should be used quickly, with any remaining portion discarded, and frozen solutions should not be used. When storing, it should be kept in a sealed container at a temperature below 25 degrees to preserve its effectiveness and prevent deterioration.
There may be respiratory issues, including hypotension, tachypnea, and recurrent respiratory distress. Serious symptoms such as coma due to fat overload syndrome may occur.
Occasionally, symptoms such as phlebitis, venous pain, bleeding tendency, recurrent venous thromboembolism, and reticulocytosis may occur. Symptoms such as hypotension, tachycardia, tachypnea, recurrent respiratory distress, and cyanosis may also appear. Nausea, vomiting, diarrhea, thirst, fever, chills, facial flushing, facial swelling, chest discomfort, unpleasant taste, and persistent erection may occur. Hyperlipidemia, fever, fat accumulation, prolonged blood coagulation time, and thrombocytopenia may also be seen.
Not everyone experiences these, and they can vary from person to person.
This medication should be stored in a sealed container at room temperature below 25 degrees. This storage method maintains the drug's effectiveness and prevents degradation.
