Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), reduction of mortality risk after myocardial infarction
This medication is used to effectively control blood pressure in patients with essential hypertension. It is also prescribed for heart failure patients who are intolerant to ACE inhibitors, especially those corresponding to NYHA class II to IV. After myocardial infarction, it helps reduce the risk of death in patients who present with left ventricular dysfunction or impaired systolic function. This drug plays a crucial role in lowering the risk of cardiovascular disease and stabilizing the patient's condition. Therefore, it is used for essential hypertension, heart failure, and reduction of mortality risk after myocardial infarction.
Essential Hypertension: It is recommended to take 80 mg orally once daily with water during meals or in between meals, ideally at the same time every day. If necessary, it can be increased to 160 mg per day, with a maximum dosage of 320 mg. Elderly patients (75 years or older) should start with 40 mg once daily. Heart Failure: 40 mg orally twice a day is recommended, with the possibility of increasing to 80 mg or 160 mg as necessary.
Safety and efficacy in children and adolescents under 18 years old have not been established, so it should not be administered.
This medication can cause serious harm to the fetus and newborn, especially during pregnancy, so it must be discontinued immediately once pregnancy is confirmed. People who are allergic to this drug or its ingredients, pregnant or nursing women, patients with severe liver impairment or cirrhosis, biliary obstruction patients, hereditary angioedema patients, and patients with severe kidney impairment should not take this drug. Elderly patients, those with elevated potassium levels, patients with liver dysfunction, valvular heart disease, cardiovascular disease, and those with volume depletion need special caution when taking this medication, and careful dose adjustments and observation of condition are important. Possible side effects include dizziness, headache, fatigue, cough, and diarrhea, while rare but serious adverse reactions such as angioedema, kidney impairment, hepatitis, hypoglycemia, and myalgia may occur. Blood pressure may drop sharply, so caution for hypotensive symptoms is necessary when starting the medication or increasing the dosage, especially for patients on dialysis or diuretics. This drug may interact with other antihypertensive medications, diuretics, potassium-sparing diuretics, non-steroidal anti-inflammatory drugs (NSAIDs), and lithium preparations, so consultation with a doctor or pharmacist is essential. Pregnant and nursing women should refrain from using this medication; however, if unavoidable, consultation with a specialist and careful monitoring are required. Safety and efficacy in children or adolescents under 18 years old have not been established, hence its use is not recommended. In case of overdose, symptoms such as hypotension, decreased consciousness, and shock may occur, requiring immediate medical attention for appropriate treatment. This medication should be kept out of reach of children, and transferring it to other containers can lead to accidents or quality degradation, so caution is advised.
Urticaria, severe swelling (angioedema), chest tightness, difficulty breathing, cold sweat, paleness, severe rash with fever, blisters, skin peeling, pain in oral/eye mucosa, shortness of breath or difficulty breathing, asthma attack, respiratory distress accompanied by cough and fever, jaundice (yellowing of skin or eyes), severe lethargy throughout the body, sudden and extremely severe headache, thunderclap headache, seizures, confusion, visual disturbances, paralysis, and coma
