Summary

Main use
Treatment of essential hypertension and chronic stable angina; adjunctive treatment of congestive heart failure
Who takes it
Adults
Adult dose
Do not increase the dose or discontinue use without a physician's instructions. Essential hypertension—Adults: Administer 12.5 mg of carvedilol orally once daily for the first two days, then administer 25 mg orally once daily. The usual dose is 25 mg orally once daily for 2 weeks. If a dose increase is needed, increase to 25 mg twice daily. The maximum dose per administration is 25 mg, and the maximum daily dose is 50 mg. Essential hypertension—Elderly: In some elderly patients, 12.5 mg once daily may be sufficiently effective. However, if the effect is insufficient after 2 weeks of treatment, the dose may be increased. Chronic stable angina: Administer 12.5 mg of this medication orally twice daily for the first two days, then administer 25 mg orally twice daily. The usual dose is 25 mg twice daily for 2 weeks. If a dose increase is needed (excluding elderly patients), the dose may be increased to 50 mg twice daily. Congestive heart failure: Before starting treatment with this medication, the standard treatment regimen already being administered (the conventional treatment of chronic heart failure with an ACE inhibitor, diuretic, and usually a digitalis preparation) should be stable. Fluid retention should also be minimized before starting treatment with this medication. Congestive heart failure: In general, the initial treatment is 3.125 mg of this medication orally twice daily for 2 weeks. If the patient tolerates this treatment well, increase the dose to 6.25 mg twice daily for 2 weeks, then to 12.5 mg twice daily for 2 weeks, and then to 25 mg twice daily. Congestive heart failure: Gradually increase the dose after taking each dose level for at least 2 weeks, up to the dose tolerated by the patient. Congestive heart failure: The effective minimum dose is 6.25 mg twice daily. The maximum dose is 25 mg twice daily for patients weighing 85 kg or less, and 50 mg twice daily for patients weighing more than 85 kg. Congestive heart failure: To control the rate of absorption, administration with meals in the morning and evening is recommended. Taking the medication with meals slows the rate of absorption and may reduce the incidence of orthostatic hypotension.

Precautions

This medication should not be used in patients with certain diseases or conditions. For example, it should not be administered to patients who are allergic to it, have very weak cardiac function, have conduction issues, have asthma or chronic lung diseases, or are pregnant or breastfeeding. Moreover, diabetic patients, patients with impaired kidney function, elderly patients, and patients with very low blood pressure should use this medication with extra caution, under careful monitoring by their physician. During the use of this medication, symptoms such as dizziness, fatigue, headache, decreased heart rate, and low blood pressure may occur; in severe cases, heart failure or conduction disturbances may develop, thus regular monitoring of cardiac function and blood pressure is necessary. Abruptly stopping the medication may worsen symptoms; therefore, dosage should be reduced gradually following a physician's instructions. Additionally, this medication has many interactions with other drugs, so it is essential to inform your physician about any other medications being taken, particularly antihypertensives, antiarrhythmics, diabetes medications, antidepressants, and alcohol. If pregnant or breastfeeding, this medication should not be used as it may harm the fetus or newborn, and safety for children and adolescents has not been established. Elderly patients may experience prolonged drug residence in the body due to decreased liver function; therefore, treatment should start at a lower dosage and be monitored closely. In cases of overdose, severe hypotension, decreased heart rate, respiratory distress, and loss of consciousness may occur, so immediate communication with healthcare professionals and appropriate treatment are required. This medication is sensitive to light, so it should be stored in a light-blocking container and kept out of reach of children. Transferring to another container is to be avoided.
