This drug improves limb ulcers and rest pain caused by chronic arterial occlusive disease (Buerger's disease, atherosclerosis). It also aids in the healing of skin ulcers in progressive systemic sclerosis and systemic lupus erythematosus. In patients with Raynaud's phenomenon, it alleviates subjective symptoms caused by peripheral blood circulation disorders and supports recovery of peripheral circulation, as well as neurological and motor function. It is used to maintain patency of the ductus arteriosus in patients with ductus arteriosus-dependent congenital heart diseases. It is also effective in improving skin ulcers caused by diabetes. Additionally, it enhances the imaging capacity in selective superior mesenteric angiography and helps maintain blood flow post-revascularization surgery. Overall, it is used to effectively improve various symptoms related to blood circulation disorders.

How it is given

Adult administration

1. Chronic arterial occlusive disease, progressive systemic sclerosis, systemic lupus erythematosus, Raynaud's phenomenon, diabetic skin ulcers: Administer 1-2 milliliters once daily (5-10 micrograms of alprostadil) either undiluted or mixed with intravenous fluids for slow intravenous infusion or drip infusion. 2. Precautions

This drug may cause apnea when administered to infants with ductus arteriosus-dependent congenital heart diseases, therefore it should only be used in facilities equipped with respiratory management equipment. Care must be taken when cutting the ampule as glass fragments may be introduced, especially when used for children or the elderly. It should not be administered to patients with severe heart failure, those with bleeding, pregnant women or women who may become pregnant, patients allergic to this drug or its components, and patients with hypersensitivity to soybean oil, soybeans, or peanuts. Caution should be exercised when administering to patients with heart failure, glaucoma, a history of peptic ulcer disease, interstitial pneumonia, kidney failure, bleeding tendencies, patients taking anticoagulants, platelet aggregation inhibitors, thrombolytics, and those with hyperlipidemia or diabetic hyperlipidemia. During treatment, side effects such as vascular pain, abnormal liver function, injection site erythema, vasculitis, headache, diarrhea, nausea, etc., may occur, so careful observation is necessary. Special observation is needed when administering to infants, as adverse reactions such as apnea, fever, and hyponatremia may occur. Anaphylactic reactions, loss of consciousness, heart failure or pulmonary edema, interstitial pneumonia, myocardial infarction, bleeding, decreased blood cells, and impaired liver function may occur; if abnormal symptoms appear, administration should be stopped immediately and appropriate treatment sought. This drug is used symptomatically for patients with chronic arterial occlusive disease, systemic sclerosis, systemic lupus erythematosus, Raynaud's phenomenon, and diabetic skin ulcers, and symptoms may recur after discontinuation, so caution is advised. Also, kidney failure, pulmonary edema, and pleural effusion may occur, so circulatory status such as blood pressure and pulse should be checked frequently, and dosing adjustments may be necessary for the elderly as their physiological function decreases. For patients with diabetic skin ulcers, basic diabetes management should continue, and if no effect is observed after 4 weeks, consider changing to another treatment. Since it has antiplatelet aggregation effects, the risk of bleeding may increase when used concurrently with anticoagulants, platelet aggregation inhibitors, and thrombolytics. It should not be administered to pregnant women or women who may become pregnant due to the risk of uterine contraction. Safety in children other than those with ductus arteriosus-dependent congenital heart disease has not been established, so caution should be taken. When mixed with intravenous fluids, this drug should be used within 24 hours, and it is safe not to mix with other medications. When cutting the ampule, orient the marking point upwards and cut downwards, and clean with alcohol swabs to prevent contamination.
