Treatment of chronic renal failure (hyperparathyroidism, metabolic bone disease) and hypoparathyroidism, osteomalacia
This medication is used to treat secondary hyperparathyroidism and associated metabolic bone diseases in patients with chronic renal failure before dialysis treatment. It is also helpful in treating renal osteodystrophy in patients undergoing hemodialysis. It is effective for hypoparathyroidism, including post-surgical, idiopathic, and pseudohypoparathyroidism. It is used for conditions like vitamin D-dependent rickets or hypophosphatemic vitamin D-resistant rickets, as well as osteomalacia. Finally, it helps in the treatment of osteoporosis, addressing various bone-related issues.
For patients before the dialysis treatment stage and children over 36 months, start with 0.25 micrograms daily, which may be increased to 0.5 micrograms daily as needed. For renal osteodystrophy (dialysis patients), start with 0.25 micrograms daily; if serum calcium levels are normal or low, administer every other day, and can be increased by 0.25 micrograms daily every 2-4 weeks as necessary. For osteoporosis patients, administer 0.25 micrograms twice daily. In cases of hypoparathyroidism, initial dosing should be 0.25 micrograms of this drug every morning. If there is insufficient response in biochemical tests and clinical symptoms, increase dosage every 2-4 weeks. Monitoring of serum calcium levels should occur more than twice weekly during dosage adjustment periods.
For children under 36 months, administer 10-15 ng per kg of body weight daily.
This medication should not be used by patients with conditions related to hypercalcemia or hypercalciuria. Patients with allergic reactions to this drug or its ingredients should also refrain from using it. Women who are pregnant or breastfeeding should use this medication cautiously, and special attention is needed for young children or patients with a history of kidney stones. This drug is an active form of vitamin D, and overdose may result in hypercalcemia, which can manifest with symptoms like nausea, vomiting, constipation, headache, insomnia, palpitations, and more. During treatment, serum calcium and phosphorus levels should be regularly monitored to maintain them within the normal range. Calcium intake should be limited to an appropriate amount, as excessive intake increases the risk of side effects. Caution should be exercised when taken with other vitamin D products or calcium supplements, as this may increase the risk of hypercalcemia. Interaction may occur particularly with diuretics or digoxin, so it is necessary to consult healthcare providers. In cases of overdose, emergency measures such as inducing vomiting or gastric lavage may be needed, and the condition should be closely monitored through blood tests and electrocardiograms. This medication requires careful dosage adjustments and attention for children, the elderly, and patients with impaired kidney function. Pregnant or nursing women should take it based on a doctor's discretion, and calcium levels in both the mother and baby should be monitored while breastfeeding. Driving or operating machinery is safe within the recommended dosage, but caution is needed in case of overdose. This medication should be stored away from direct sunlight and moisture, below 25 degrees Celsius, and kept out of reach of children.
