Efficacy & effects

Effect of improving acid-base balance in patients with chronic renal failure

View approved original text

The alkaline solution of dialysate used for artificial renal dialysis in patients with chronic renal failure helps regulate electrolytes and acid-base balance in the body. This medication is used during artificial kidney treatment to remove waste and excess fluid from patients with reduced kidney function. In particular, it helps improve acidosis and maintain the body's acidity balance. This stabilizes the patient's overall condition and enhances therapeutic effects. Therefore, it is essential for the artificial kidney treatment of chronic renal failure patients.

How it is given

Adult administration

An appropriate acidic solution for artificial dialysis should be selected based on the patient's condition and the type of artificial kidney being used, then diluted with this solution and purified water.

Precautions

This medication is a dialysis solution used for patients with chronic renal failure and contains sodium bicarbonate. It should be used with caution in patients with severe liver failure or severe diabetes that may affect acetate metabolism, as improper metabolism of acetate can lead to symptoms of hypotension. Patients taking digitalis preparations should also be cautious, as this medication can lower serum potassium levels, posing a risk of digitalis toxicity. Patients with elevated serum calcium and phosphorus levels before dialysis should be careful when using this medication, as complications like metastatic calcification may occur. Patients with insufficient calorie intake, dietary restrictions due to diabetes, or those using hypoglycemic agents should be cautious, as they may develop hypoglycemia. During the use of this medication, regular monitoring is necessary due to the potential occurrence of complications related to calcium metabolism, such as hypoglycemia, hypercalcemia, osteoporosis, and osteomalacia. Symptoms of dialysis disequilibrium syndrome may occur, presenting with headaches, nausea, vomiting, convulsions, and altered consciousness. Appropriate measures should be taken if symptoms arise. Because this medication can affect the cardiovascular system, symptoms of hypotension or shock may occur; in such cases, dialysis should be discontinued or efficiency reduced, and fluids or antihypertensive agents should be administered. Rarely, hypertension may occur, necessitating the administration of antihypertensive agents. Pregnant women or those who may become pregnant should only use this medication when the benefits of treatment outweigh potential risks, as its safety has not been established. Safety in children has not been established, so caution is advised when used in pediatric cases. Elderly patients may develop bone-related complications with long-term use; therefore, regular clinical examinations and administration of active Vitamin D3 as needed are recommended. This medication must be diluted to the specified concentration and can cause side effects such as headaches, palpitations, abnormal blood pressure, and altered consciousness if the concentration is too high or too low. Only purified water should be used for dialysis diluents, and care should be taken regarding potential dialysis membrane damage and bleeding tendencies. This medication should not be used for injection or peritoneal dialysis and must be used immediately after preparation. Insoluble particles may develop if it is used more than 2 hours after mixing. It is advisable to maintain the solution at body temperature. When used with acidic solutions containing glucose, significant fluctuations in blood sugar may occur, necessitating immediate appropriate measures in case of hypoglycemia or hyperglycemia symptoms. If weight gain or elevated blood pressure occurs, it should be managed through fluid removal or weight control. This medication is a dialysis solution with low concentrations of potassium, calcium, and magnesium, suitable for patients with hyperkalemia, hypermagnesemia, or hypercalcemia. Regular blood tests to monitor electrolytes, acid-base balance, renal function, and blood glucose are important.
