This medication is mainly used when there is a deficiency of fluids and electrolytes, especially when sodium or chloride is deficient. It is also utilized as a solvent for dissolving or diluting other injections. Thus, it helps in the replenishment of dehydration or electrolyte imbalance and in the preparation of injections.

How it is given

Adult administration

Typically, Sodium Chloride is administered subcutaneously, intravenously, or via a drip as 20 to 1,000 mL. The dosage and administration rate should be adjusted according to age, weight, and symptoms.

Precautions

This medication must be handled very carefully when cutting the glass container of the ampule to avoid the risk of glass fragments. Special caution is needed, especially when administering to children and the elderly. This medication should not be administered to patients with hypernatremia, those with excess fluid in the body, and patients with cirrhosis accompanied by edema or ascites. Patients with cardiovascular or circulatory issues, kidney disorders, low protein levels in the blood, hyponatremia, hypokalemia, hyperchloremic acidosis, patients with peripheral or pulmonary edema, preeclampsia, those with certain hormonal abnormalities, and children and the elderly should be administered this drug with caution. Administering this drug in large amounts or rapidly may cause side effects such as congestive heart failure, edema, acidosis, electrolyte abnormalities in the blood, febrile reactions, injection site infections, local pain, venous irritation, thrombosis, extravasation of medication, or hypervolemia. If any abnormal symptoms appear, administration should be stopped immediately, and the patient’s condition should be reassessed, taking appropriate action. During the use of this medication, regular clinical assessments and laboratory tests are necessary to monitor fluid balance, electrolyte concentrations, and acid-base balance changes. Extra caution is required when administering to pregnant women, nursing mothers, women of childbearing age, newborns, infants, children, and the elderly. Notably, the container of this medication is made of PVC material containing DEHP, a plasticizer that may affect reproductive development in animal studies. However, the actual risk to humans is very low, making the use of this medication safer. The elderly may have reduced bodily functions; hence, the infusion rate should be slow, and the dose should be reduced. Children may have an impaired ability to regulate fluids and electrolytes, so careful monitoring of blood electrolyte levels is essential. Administration of this medication to patients with acute fluid loss may lead to hyponatremia, which can result in headaches, nausea, seizures, drowsiness, coma, brain edema, and, in severe cases, death, necessitating swift emergency response. Before administration, check for any change in the solution color, presence of precipitates, insoluble substances, or crystallization, and disinfect the patient’s skin and the instruments used to prevent infection. Particularly in colder seasons, warm the solution to body temperature before use, and it should be used immediately after opening or mixing with other medications, with any remaining solution discarded. Finally, this medication should be administered slowly, and when preparing, it is essential to confirm that it is suitable for use as a solvent for dissolving or diluting.
