Amino acid supply is used to improve and support the nutritional status of patients in conditions such as hypoproteinemia, malnutrition, and preoperative or postoperative states.
Dosage and Administration
Adult dosage
When administered via peripheral vein, it is generally infused as a single dose of 200 to 400 mL. The infusion rate is approximately 120 minutes per 200 mL (about 25 drops per minute), and around 10 g within 60 minutes is desirable for optimal utilization in the body. When administered via central venous route, it is generally continuously infused at 400 to 800 mL per day using a high-calorie fluid method. The dosage may be appropriately adjusted according to age, symptoms, and body weight.
Children's dosage
When administered via peripheral vein, it should be infused slowly in pediatric patients.
Precautions
This medication must only be used as directed by a physician. It should not be administered to patients with hepatic coma or those at risk for hepatic coma. Patients with severe renal impairment or elevated blood urea nitrogen levels should also not use this medication. It must not be administered to patients with metabolic disorders affecting nitrogen utilization, cachectic heart failure, or pulmonary edema. Patients with oliguria or anuria should not use this medication. If sodium and chloride ions are present, it should not be administered to patients with hypernatremia, hyperchloremia, or alkalosis. Patients with a history of myocardial infarction should not use this medication. Caution should be exercised in patients with severe acidosis, congestive heart failure, edema due to sodium retention, or hyperkalemia or potassium retention. Patients with liver or kidney impairment should also be monitored carefully, and those allergic to sulfites should be cautious as severe allergic reactions may occur. Rarely, hypersensitivity reactions such as skin rashes may occur; if such symptoms arise, administration should be stopped immediately. Occasionally, gastrointestinal symptoms such as nausea or vomiting may occur. Cardiac-related symptoms may include chest discomfort, palpitations, rapid heartbeat, and elevated blood pressure. Rapid large dosing may lead to acidosis in the body, so caution is required. Symptoms such as chills, fever, headache, shortness of breath, and in severe cases, respiratory arrest, shock, bronchospasm, urinary spasms may occur. Metabolic complications may include acidosis, hypophosphatemia, alkalosis, hyperglycemia, diabetes, dehydration, hypoglycemia, elevated liver enzymes, vitamin imbalance, and electrolyte imbalances. Patients with impaired liver or kidney function may have increased blood urea nitrogen levels. Patients with liver failure may experience symptoms like amino acid imbalance, metabolic alkalosis, systemic nitrogen retention, hyperammonemia, or coma. Special care should be taken when administering large doses or using it with other fluids containing electrolytes to maintain electrolyte balance. Overdosage may lead to metabolic alkalosis; therefore, formulations containing acetate ions should be used cautiously. Patients with a history of kidney disease, lung disease, or severe heart disease should adjust the rate and amount of infusion to prevent fluid overload. Diabetic patients should monitor blood sugar levels frequently during use. Concomitant use with certain antibiotics such as tetracycline may reduce this medication's protein-sparing effects. Pregnant women should only use this medication when the benefits outweigh the risks, as safety has not been established. When administered to children, it is advisable to maintain lower concentrations due to potential immature liver function and to monitor blood ammonia levels frequently. Elderly patients may experience decreased physiological function, so it is important to administer slowly and reduce the dosage accordingly. This medication contains electrolytes, and caution should be taken regarding electrolyte balance during excessive administration; if the infusion rate is too rapid, chills or vomiting may occur. It is recommended to warm the solution to body temperature before use and to prepare and handle it in a sterile manner to reduce the risk of infection. Before administration, check that the solution is completely clear, and do not use if there is discoloration or leakage.
