Prevention and treatment of cerebrovascular spasm-related ischemic neurological disorders following aneurysmal subarachnoid hemorrhage
This medication is used to prevent and treat ischemic neurological disorders caused by vasospasm after aneurysmal subarachnoid hemorrhage. It is effective in improving conditions where blood supply to the brain is insufficient due to sudden constriction of blood vessels after hemorrhage, thus preventing damage to neurological functions. Therefore, it is used as a treatment to protect the brain health of patients and support their recovery.
(Oral: tablets) ○ Adults Swallow whole without chewing, with a small amount of liquid, regardless of meals. 1. Ischemic neurologic disorders due to cerebral vasospasm: After administering nimodipine injection for 5-14 days after hemorrhage, administer nimodipine 60㎎ orally 6 times daily at 4-hour intervals for 7 days. 2. Cerebral dysfunction: Administer 30㎎ of this medicine 3 times daily. Maintain an interval of at least 4 hours between doses. Do not use the oral and injectable formulations concomitantly. Adjust the dose as appropriate according to age and symptoms. (Injection) 1. Intravenous infusion Start at 1㎎(5㎖) per hour (approximately 15㎍/kg of body weight) and administer over at least 2 hours. After 2 hours, the rate may be increased to 2㎎(10㎖) per hour (approximately 30㎍/kg of body weight), provided the patient tolerates the medicine and, in particular, no marked decrease in blood pressure occurs. In patients weighing less than 70㎏ or with unstable blood pressure, treatment may be started at 0.5㎎(2.5㎖) per hour. Reduce the dose if the medicine is not tolerated. Infuse with fluids and blood through a catheter using an infusion pump in an extracorporeal circulation system. To adequately dilute this medicine, the concomitant infusion volume should be at least 1,000㎖ per day. Continue administration during anesthesia, surgery, and angiography. Because the active ingredient in this medicine is adsorbed onto PVC, use polyethylene tubing. 2. Intra-arachnoid administration Dilute 1㎖ of this medicine in 19㎖ of Ringer's solution. Warm the diluted medicine to approximately body temperature (37℃) and administer by infusion during surgery. 3. Duration of administration 1) Prevention Start intravenous administration within 4 days after hemorrhage and continue during the period when vasospasm is likely to occur, i.e., until 10-14 days after hemorrhage. Then continue oral administration for 7 days at 60㎎ per dose, 6 times daily at 4-hour intervals. 2) Treatment If neurologic disorders due to vasospasm have already developed after hemorrhage, start treatment as soon as possible and administer intravenously for at least 5 to 14 days. After intravenous administration, continue oral administration for 7 days at 60㎎ per dose, 6 times daily at 4-hour intervals. If the cause of the hemorrhage is treated surgically during treatment or prevention, continue intravenous administration for at least 5 days after surgery. The total duration of continuous administration must not exceed 21 days. Do not use the injection and oral formulations concomitantly. Adjust the dose as appropriate according to age and symptoms.
