Summary

Main use
Treatment of hypertension; adjunctive treatment of congestive heart failure; treatment of early diabetic nephropathy
Who takes it
Adults
Adult dose
1. Hypertension 1) Patients not receiving diuretics Adults: The initial dose of lisinopril is 10㎎ orally once daily (adjust according to the blood pressure response). The maintenance dose is 20-40㎎ once daily. Measure blood pressure immediately before dosing to determine whether satisfactory blood pressure control is maintained for 24 hours; increase the dose if a satisfactory effect is not maintained for 24 hours (doses up to 80㎎ have been used, but do not produce a greater effect). If blood pressure is not controlled with this drug alone, a low-dose diuretic may be given concomitantly (concomitant administration of 12.5㎎ hydrochlorothiazide has a synergistic effect); in this case, it may be necessary to reduce the dose of this drug. 2) Patients receiving diuretics Adults: When this drug is administered concomitantly with a diuretic, symptomatic hypotension may occur after administration. If possible, discontinue the diuretic for 2-3 days before starting this drug. Adjust the dose according to the blood pressure response. If the diuretic cannot be discontinued, administer an initial dose of 5㎎ under a physician's supervision for at least 2 hours and for at least 1 hour until blood pressure is stabilized. 3) Elderly patients In general, blood pressure responses and adverse effects have been found to be similar in younger and elderly patients receiving the same dose of this drug. However, pharmacokinetic studies have shown that the maximum plasma concentration and area under the plasma concentration-time curve are 2 times higher in elderly patients; therefore, take particular care when adjusting the dose. 4) Patients with renal impairment The following doses are administered according to creatinine clearance and may be increased until blood pressure is controlled or to a maximum of 40㎎ per day. Normal renal function-mild impairment: >30㎖/min (serum creatinine <3㎎/㎗), initial dose 10㎎/day Moderate-severe impairment: ≥10, ≤30㎖/min, initial dose 5㎎/day Dialysis patients: <10㎖/min (usually during hemodialysis), initial dose 2.5㎎/day ※Adjust the dose and dosing interval according to the blood pressure response. 2. Congestive heart failure Adults: Use as adjunctive treatment for congestive heart failure in combination with a diuretic or digitalis. The initial dose is 2.5㎎ once daily, and the maintenance dose is 5-20㎎ once daily. 3. Renal complications of diabetes mellitus Treatment is started at 2.5 milligrams once daily, and the dose is adjusted to the effective dose. In normotensive patients with insulin-dependent diabetes mellitus, the dose is 10 milligrams once daily; it may be increased to 20 milligrams once daily if needed to lower the seated diastolic blood pressure to below 75mmHg. In hypertensive patients with non-insulin-dependent diabetes mellitus, the same dosage regimen is used if needed to lower the seated diastolic blood pressure to below 90mmHg.

This medication is used to treat hypertension. It is also used as an adjunctive treatment in patients with congestive heart failure in conjunction with diuretics or digitalis. Additionally, it is effective in early nephropathy in diabetic patients showing kidney complications. It is used in patients with insulin-dependent diabetes when blood pressure is normal and in non-insulin-dependent diabetes cases when accompanied by hypertension. It helps improve symptoms in patients due to its therapeutic effects on these conditions.

Precautions

This medication should not be used in certain patients. For example, it should not be administered to patients undergoing hemodialysis, receiving specific lipid-lowering treatments, having allergies to this medication or similar drugs, with a history of angioedema, those who have received kidney transplants, pregnant or breastfeeding women, and children. Furthermore, patients with severely impaired kidney function, the elderly, patients with severe hypertension, those with significant proteinuria, patients with autoimmune diseases, congestive heart failure, or renal artery hypertension should use this medication cautiously with necessary dose adjustments and regular monitoring. This medication can cause a variety of side effects, including mild symptoms such as fever, sweating, flushing, abdominal pain, nausea, diarrhea, and cough. In severe cases, it may cause hypotension, arrhythmia, myocardial infarction, angioedema (especially facial, lip, tongue, or laryngeal swelling causing breathing difficulties), renal failure, liver dysfunction, blood abnormalities, and serious skin diseases. If any of these symptoms occur, medical consultation should be sought immediately. Prior to and during administration, kidney function and electrolyte levels must be monitored, especially in patients with impaired kidney function, who require careful management. Blood pressure may drop rapidly, so treatment should start at a low dose, with close observation of the patient's condition. This medication can interact with other antihypertensives, diuretics, potassium-sparing medications, lithium, alcohol, some anti-inflammatory drugs, and immunosuppressants, so if taking other medications, this should be communicated to a physician or pharmacist. During pregnancy, especially after the second trimester, there can be serious risks to the fetus, so if pregnancy is planned or confirmed, this medication should be discontinued immediately, and alternative treatments should be sought. Nursing mothers should also avoid this medication, and if unavoidable, breastfeeding should be discontinued. In case of overdose, symptoms such as hypotension, shock, electrolyte abnormalities, respiratory distress, and arrhythmia may occur, requiring immediate emergency treatment and appropriate care and monitoring in a hospital. This medication should be stored out of reach of children and should not be transferred to other containers. Additionally, if acute allergic reactions or symptoms of angioedema occur while taking this medication, it should be discontinued immediately, and medical personnel should be informed.
