Relief of severe and moderate acute chronic pain (pain from cancer, diagnostic and postoperative pain)
This medication is used for the relief of severe and moderate acute and chronic pain, such as pain arising from cancer and pain occurring after diagnosis or surgery.
As tramadol hydrochloride, 50 to 100 mg is administered via intravenous or intramuscular injection, and it may be repeated every 4 to 5 hours as needed. The maximum daily dose can be up to 400 mg.
This medication may increase the risk of seizures if taken in excess of the recommended dose. It should be used only when necessary in patients with a history of seizures or those at risk for seizures. Long-term use may lead to psychological and physical dependence; thus, patients with a history of substance abuse or dependency should only use it under strict supervision for a short duration. Taking this medication with alcohol or central nervous system depressants can cause severe sedation, respiratory depression, coma, or even death, so co-administration should be avoided or approached with great caution. Patients who metabolize this medication rapidly due to genetic factors are at a higher risk for serious side effects like respiratory depression, so dose adjustment and monitoring of symptoms are necessary. It should not be administered to children under the age of 12 or to children under 18 years of age who have undergone tonsillectomy and adenoidectomy. Adolescents aged 12 to 18 should also avoid using it if special risk factors are present. When using the injection, care should be taken to cut the container carefully to prevent glass fragments from mixing in, especially when administering to children or the elderly. It should not be used in cases of acute alcohol intoxication, central nervous system drug intoxication, severe respiratory depression, head injury patients at risk for altered consciousness, patients on MAO inhibitors, those with severe renal or liver impairment, pregnant or breastfeeding women, and patients with certain genetic disorders. Patients using morphine, those with liver impairment, alcohol users, and those with a history of seizures should be administered cautiously, and care is needed as the risk of dependency may increase in individuals with a history of substance abuse or mental illness. During treatment, symptoms such as drowsiness and dizziness may occur, and driving or operating dangerous machinery should be avoided. Tolerance may develop, requiring dosage adjustments; therefore, sudden discontinuation should be avoided, and tapering is recommended. In cases of overdose, severe respiratory depression and seizures may occur, necessitating immediate emergency treatment; naloxone may be used as an antidote. This medication can interact with various drugs, so if other medications are being taken, consulting with a doctor or pharmacist is essential, especially regarding central nervous system depressants, antidepressants, and MAO inhibitors. Pregnant and breastfeeding women should not use this medication, and if breastfeeding, the baby should be monitored for symptoms of sedation or respiratory depression. Elderly patients may have increased blood levels, so the maximum daily dose should be limited, and while storing it, it should be kept out of reach of children.

This medication is sensitive to light and should be stored in a container that blocks light. Additionally, it is important to store it in a cool room temperature to maintain its effectiveness.
