Treatment of epilepsy (including tonic-clonic and partial seizures) and digitalis-induced arrhythmias
This medication is used to treat various types of seizures in patients with epilepsy, including tonic-clonic seizures (grand mal seizures), partial seizures (focal seizures), and psychomotor seizures. The injectable form is especially effective for managing status epilepticus and for cases where oral suppression of seizures is difficult, such as in patients with altered consciousness or during postoperative recovery. This medication is also used to treat digitalis-induced ventricular arrhythmias when first-line therapy is ineffective. It aids in seizure control and improvement of symptoms of arrhythmia.
The dosage should be adjusted according to the severity of seizures and the patient's tolerability. Measure the blood concentration if necessary. This medicine should not be administered once daily; it must be given in divided doses. 1. Adults: Administer 100 mg of phenytoin orally 3 times daily. The maintenance dose is 300 ~ 400 mg of this medicine daily in divided doses. The daily dose may be increased to 600 mg of this medicine. (Injection) 1. Status epilepticus or seizures that are difficult to control with oral administration Adults: Slowly administer 125 ~ 250 mg of phenytoin sodium by intravenous injection at a rate not exceeding 50 mg/min. If seizures cannot be controlled with this dose, administer an additional 100 ~ 150 mg of this medicine after 30 minutes or consider other treatment. When convulsions have ceased and consciousness has recovered following administration of this medicine, switch to oral administration. 2. Digoxin-induced arrhythmia for which first-line treatment is ineffective Adults: Administer an initial dose of 3.5 ~ 5 mg/kg of body weight of phenytoin sodium by intravenous injection. Repeat once if necessary. This medicine must be administered slowly by intravenous injection at a steady rate not exceeding 50 mg/min. Adjust the dose as appropriate according to age and symptoms.
2. Children: Administer 5 mg/kg of body weight of phenytoin in 2 ~ 3 divided doses. The maintenance dose is 4 ~ 8 mg/kg of body weight of this medicine daily in divided doses. Up to 300 mg of this medicine may be administered daily.
This medication is used for epilepsy treatment, and patients and caregivers should closely monitor for signs of suicidal thoughts, suicidal behavior, or other mental health issues like depression during treatment. When administered as an injection, inflammation or pain at the injection site, skin discoloration, and in severe cases, skin necrosis or tissue damage may occur, necessitating caution. Patients with hypersensitivity to this medication or those with certain heart diseases or blood disorders should not take it, and it is essential to consult healthcare professionals regarding the use of this medication with other drugs, as it may alter drug concentrations or the risk of side effects. Patients with impaired liver or kidney function, the elderly, and those with diabetes must use this medication cautiously, and treatment should be immediately stopped, and a doctor consulted if severe skin reactions occur. During treatment, drowsiness, dizziness, and decreased motor ability may occur, so patients should avoid driving or operating hazardous machinery, and doses should not be abruptly reduced or discontinued as this may exacerbate seizures. Use during pregnancy may increase the risk of congenital anomalies; therefore, women who are pregnant or planning to become pregnant must consult a healthcare professional before taking this medication and should employ effective contraception. Regular liver, kidney, blood tests, and vision checks are necessary, and maintaining appropriate dosing through drug concentration measurements is important. In the event of an overdose, serious symptoms such as coma, lowered blood pressure, and difficulty breathing may occur, requiring immediate emergency care.
