Prevention and treatment of thrombosis and embolism, anticoagulant effect
This medication is used for the prevention of thrombosis and the treatment of postoperative thrombosis and pulmonary embolism. It is also effective for the prevention and treatment of embolic atrial fibrillation, diagnosis and treatment of acute and chronic coagulation disorders. It is used to prevent blood coagulation during vascular and cardiac surgeries, and to prevent cerebral thrombosis caused by cerebral hemorrhage. It is also applied to prevent blood coagulation during transfusions, extracorporeal circulation, and dialysis, as well as in the treatment of coronary artery occlusion and peripheral vascular embolism. Overall, this drug plays a very important role in preventing blood coagulation and treating related diseases.
Typically administered intravenously or added to blood during extracorporeal circulation. 1. Extracorporeal circulation 1) Artificial kidney (1) Systemic heparinization: Administer 1,000 to 5,000 IU of heparin sodium before starting dialysis, and then continuously administer 500 to 1,500 IU per hour or intermittently add 500 to 1,500 IU every hour post-start. (2) Local heparinization
This drug is very potent and should only be used under strict medical supervision. Particular caution should be exercised when administering this drug to patients with conditions that carry a high risk of bleeding. Bleeding can occur in almost any part of the body and can sometimes result in life-threatening severe bleeding. Rarely, bleeding can occur behind the adrenal glands, ovaries, or in the peritoneal cavity, leading to serious complications such as acute adrenal insufficiency. The benzyl alcohol component in this drug can cause fatal respiratory issues in premature infants, so it should not be used in newborns or preterm infants. It should not be administered to patients with known allergies to this drug or its ingredients, or to those with abnormal blood coagulation test results. Patients with bleeding or bleeding tendencies, severe liver or kidney disease, or those who have recently undergone brain or eye surgery should not use this drug. It is also contraindicated in patients who have received spinal or epidural anesthesia or have undergone lumbar puncture. Those with a history of thrombocytopenia or sensitivity to benzyl alcohol, especially in newborns and preterm infants, should not be treated with this medication. Caution should be exercised in using this drug in patients with consumptive coagulopathy, catheter insertion, women in late pregnancy, patients with severe retinal disease, impaired liver or kidney function, children, and the elderly. This drug can cause various side effects such as bleeding, thrombocytopenia, elevated liver enzymes, allergic reactions, skin issues, osteoporosis with long-term use, and decreased kidney function, so symptoms should be closely monitored during usage. In particular, thrombocytopenia can lead to severe thrombosis, thus platelet counts should be regularly evaluated, and usage should be stopped immediately if anomalies are found. Blood coagulation function should be monitored during treatment, and dosages adjusted accordingly; if bleeding occurs, administration should be temporarily suspended. Abruptly stopping the medication can increase the risk of thrombosis, therefore the dosage should be gradually reduced. If there is a high risk of bleeding or a rapid decrease in platelet count, the antidote protamine sulfate can be used to neutralize the effects of this drug. This medication can interact with multiple drugs, therefore caution is warranted when used in combination with acetylsalicylic acid, non-steroidal anti-inflammatory drugs, anticoagulants, etc., due to increased bleeding risks. During pregnancy, caution should be taken due to the risk of uterine and placental bleeding, and breastfeeding women also need to be careful because of the benzyl alcohol component. The safety for children has not been sufficiently established, and risks are higher for the elderly, therefore even greater caution should be exercised in these groups. In cases of overdose, bleeding may intensify, so medical personnel should be notified immediately and an antidote should be administered. During treatment, intramuscular injections should be avoided, and this drug should not be mixed and injected with antihistamines.
